Job summary

The vacancy is for the Band 7 Speech and Language Therapy Lead for a large intermediate care team based at Homerton Healthcare NHS Trust.

You will see patients with a variety of impairments, working closely with your therapist colleagues and other team members within the Integrated Independence Team, mainly with Home Treatment and Reablement but also with Rapid Response and the Virtual Frailty Ward as the service requires.

The Integrated Independence Team (IIT) is an integrated health and social care service provided jointly by Homerton University Hospital Foundation Trust and London Borough of Hackney. The service places the user at the heart of the care planning process, and aims to promote independence, deliver outcome focused care, and provide a seamless journey through the care system.

We are an innovative, exciting, friendly and supportive team, with various sub-teams coming under the umbrella of IIT - such as Rapid Response, Home Treatment and Reablement Team, Virtual Frailty Ward and Rapid Care.

Main duties of the job

The post holder is a senior member of the team and responsible for:

  • Provision of the specialist Speech and Language Therapy component of multidisciplinary intervention to complex clients of all disabilities within Intermediate care. The post is based in the hospital with therapy being carried out in the community.
  • Act as an expert resource and coordinate SLT delegated responsibilities and the delivery of SLT services within the Home Treatment and Reablement elements of the service, to enable a high quality, client focussed service ensuring staff are meeting the service standards and any service related issues are fed back to the team manager.
  • Contribute to clinical supervision and appraisal of staff within the service where appropriate. This will include assistant staff and other professions in relation to the role of the Speech and Language Therapist.
  • Supervising and teaching Speech and Language Therapy students on placement.
  • Actively contribute to the strategic direction and development of SLT and the Integrated Independence Team, including initiating developments and taking a lead on projects as required in line with Trust and London Borough of Hackney objectives, local policy, client needs and government agenda.
  • The post holder will be working in a challenging socioeconomic environment involving clients who have highly complex social problems and those from culturally diverse backgrounds.
