Job summary

This is a full-time, permanent post employed by the Homerton Healthcare NHS Trust. The post holder will be based in the palliative care service at the Homerton hospital site, which provides specialist palliative and end-of-life care to patients and people important to them at the hospital and Mary Seacole Nursing Home.

The post holder will have clinical and managerial responsibility for a defined clinical portfolio across the Trust. The post holder will oversee the development of the specialist palliative care service that has a sustainable and measurable impact on service-users' outcomes and experience. The post-holder will provide leadership to the multi-disciplinary Palliative Care Team and be accountable for ensuring the highest standard of clinical care is provided across the Trust. The Team Leader will also act as an expert practitioner in palliative care and exercise a high degree of personal and professional autonomy. The post-holder will also act as an expert resource on matters relating to palliative and end-of-life care to health care professionals throughout the Trust.

Main duties of the job

  • To oversee the delivery of high quality, safe and effective care focused on improving outcomes and experiences for adult patients with specialist palliative care needs, and their families and carers
  • To facilitate the development of palliative care at clinical, Trust and Network level, with the Nurse Consultant and other members of the multidisciplinary team
  • To lead on workforce development and capacity building in palliative and end-of-life care across the Trust in collaboration with the Nurse Consultant and the wider teams
  • To play a key role in ensuring the principles of clinical governance are implemented at a local level within the local area and across the wider directorate; ensuring the implementation of the Palliative and End-of-Life Care Strategy
  • To act as a clinical expert for palliative and end of life care, providing expert advice at clinical and strategic level
  • To have continuous responsibility for the service, providing visible professional leadership and line management for the specialist palliative and end-of-life care team
  • To manage budgets and resources efficiently and effectively
  • Champion a culture of learning and quality improvement within the service and Trust
  • Be an ambassador for palliative and end of life care across the Trust
