Job summary
An exciting opportunity has arisen to join a new, innovative and dynamic team within our expanding frailty virtual ward and rapid response team. We are looking for experienced and motivated senior therapist (Band 7) who are passionate about service development and providing high standard of patient centered care. You will be working closely within a Multi-Disciplinary Team of staff including Doctors, senior practitioner, Occupational Therapists, Physiotherapists and HCA's, all of whom are committed to maintaining independence, promoting self-care and assisting patients in their usual place of residence. At times this post may require you to work autonomously, when working across the Hackney borough into patients own homes and in an inpatient hospital setting.
Main duties of the job
- Provide a rapid response, senior occupational therapy/physiotherapy assessment and treatment of complex patients providing specialised high-quality care and support for patients in their own home to prevent unnecessary admissions and/or facilitate timely discharge from hospital/ other healthcare settings under direct or indirect supervision of senior medical colleagues.
- Participate in service development within Frailty Virtual Ward service set up, audits and outcome measures in relation to Frailty Virtual Ward Occupational Therapy or Physiotherapy practice and patient care.
- Work in a challenging socioeconomic environment involving patients who have highly complex social problems and those from culturally diverse backgrounds.
- Deputise for Band 8A Virtual Ward Lead in their absence taking responsibility for operationally managing allocated parts of the service.
