Job summary
Due to the planned retirement of the current post holder, the role of Team Leader has arisen in the therapies-led 'Integrated Independence Team', a Section 75 service offering in-reach into hospital and intermediate care in the community. The team comprises of a number of Pathways, including Rapid Response, Home Treatment, Reablement, and Rapid Care, as well as a single point of access (SPA) for referrals and a morning falls service in conjunction with paramedics. Social workers employed within IIT work across all the the pathways.
The service is fast-paced but highly successful, contributing to the Trusts efficient, safe discharge processes for the vulnerable adult residents of City and Hackney, and commitment to post-discharge therapeutic rehabilitative care. The Homerton has one of the lowest length of stays in London and we are proud to be a part of this.
Main duties of the job
The role is to lead on the delivery and support of the Home Treatment and Reablement offer, which delivers rehabilitation and care to local residents for a period of up to 6-weeks.
In conjunction with the Rapid Response and SPA team leader, the successful applicant will deputise for the Head of Service in their absence, but also provide clinical support for complex service users as required.
This is a leadership role, but one which may also provides an opportunity for the post holder to maintain their advanced clinical skills . As a Trust employee the successful postholder would have access to a range of development opportunities, and be supported in this by a personalised plan developed at appraisal.
