Job summary

The Apprentice ACP will act as a professional resource for the multi professional teams, patients and carers within their scope of practice and competence. They will ensure that evidence based practice is inherent in all aspects of care and treatment The Apprentice ACP will understand professional accountability and legal frameworks for advanced practice The Apprentice ACP will be professionally accountable and legally responsible for all aspects of own work, including the management of patients in their care that are within the competencies of the Apprentice ACP training programme. The Apprentice ACP will demonstrate safe practice in line with the Trust Quality, Safety and Risk Management Policies, Child Protection and Safeguarding Vulnerable Adults Policies, Consent and assent regulations and the PAHT Do Not Attempt Resuscitation Policy all of which must be in line with the Apprentice ACP training programme.

Main duties of the job

The post holder will be undertaking Apprentice Advanced Clinical Practitioner (ACP) training programme. They will undertake a period of full or partial supervised practice that will support the completion of clinical competencies relevant to the Band 7 role in order to progress into a Band 8A Advanced Clinical Practitioner. They will be working towards becoming an integral member of the medical team working collaboratively with nursing and allied healthcare professionals undertaking advanced clinical practice that has traditionally been within the remit of medical staff with an appropriate level of supervision until deemed competent. The Apprentice ACP will observe advanced practices and work fully supervised until assessed as competent, within agreed protocols and guidelines in history taking, examination, assessment, diagnosis, and treatment of a diverse and undetermined caseload of critically or acutely ill or injured patients who may have highly complex acute presentation to the more minor common illnesses and injuries. The post will have a primary location but may extend and influence across PAH including adult inpatient wards working at all times under an appropriate level of supervision as part of the training programme.

Clinical Practice

With support for the Consultant, be responsible for a caseload of patients and managing nurse led review clinics.

Assess, diagnosis and treat patients, develop treatment plans within set guidelines, and record clinical findings using the department information systems, when necessary collaborate with the multi-professional and/or refer to the senior clinical colleagues as appropriate.

Challenge practice so that the best and most appropriate treatment is given to the patient; maintaining and monitoring high standards of care, recognising and responding proactively to patients’ changing clinical needs.

Maintain professional competence, authority and credibility by undertaking regular clinical sessions in line with your job plan and being highly visible to patients, carers and staff, actively seeking feedback about the quality of services. Collaborate in the establishment and monitoring of protocols/care pathways.

Actively participate in case discussions and multi-disciplinary team meetings, liaise with inter hospital departments/personnel across organisations and professional boundaries.

Undertake non-medical independent prescribing, in accordance with policies, current legislation and Patient Group Directions (PGD’s).

Supervise and support junior members of staff in the delivery of patient care.

Identify deteriorating patients and take the appropriate action to manage the situation, liaising with the multi-disciplinary team and external care providers as appropriate.

Have accountability for the correct administration, usage and storage of medicines including host cost drugs according to the trust policy to include the safe administration of medicines, controlled, intravenous and cytotoxic drugs.
