Job summary

This role supports the delivery of high-quality patient care by providing rehabilitation support to cardiology patients. To work as part of a multidisciplinary team assisting the cardiac rehabilitation CNS and ACP with all aspects of the patient journey throughout the phases of cardiac rehabilitation, ensuring kind, safe, and compassionate care for patients.

Clinical

  • In consultation with the clinical nurse specialist respond to patient cardiac rehabilitation needs and liaise with other members of the multidisciplinary team as appropriate

  • Promote, monitor and maintain a healthy, safe and secure environment for patients, families and staff

  • Visiting other ward areas, collecting information from patient records and with training educating patients with a basic level of rehabilitation advice providing them with enough information for discharge for the Cardiac Rehabilitation Nurse to follow up

  • With training review patient’s clinical notes and be able to interpret data for minap audit including the normal parameters when recording results

eg blood results, observations, cholesterol, glucose levels and ambulance data.

  • To assist the clinical nurse specialist to meet the needs of the patients and service

  • To undertake appropriate referrals to members of the multi-disciplinary team under direct supervision of a registered practitioner

  • Be aware of your own responsibilities with regard to emergency situations that may occur

  • Be able to answer basic rehabilitation questions and queries over the telephone with direct guidance from a registered practitioner.

  • Ability to maintain patient safety at all times with training and knowledge of cardiac conditions

  • Provide effective communication skills with patients, families and all MDT

  • Ability to liaise with bereavement team and coroner if required for data purposes

  • Learn with training to interoperate medications from medical records for the MINAP audit

 

Administrative

 

  • To answer the telephone and refer enquiries to the clinical nurse specialist as appropriate and supports service users with information etc

  • Assist with auditing and managing data

  • Receive and process referrals form Ward level staff, Consultants, GPs and MDT to the cardiac rehabilitation service via Alex health systems

  • Assist with administrative duties to ensure that appropriate notes are available and that patients flow smoothly through the department

  • Use of Microsoft computer software and ability to data entry, competent with email and data analysis with training

  • Able collection of MINAP data, and using national spine data bases

  • Booking out patient appointments and coordination of post PCI clinic and ANP Adhoc clinic

  • In putting of data into national databases

  • Ensure that confidentiality and data protection is adhered to when entering and processing data through the patient journey of rehabilitation

  • Maintain and order agreed stock levels of educational material, photocopying leaflets, and keep all the areas tidy

  • Referral of patients to external services (Cardiac Rehabilitation Services across UK)

  • Help to maintain the cleanliness of the environment

  • To be responsible for contacting and liaising with the Works Department

  • To be responsible for the cleaning, maintenance and storage of all equipment

  • Run errands to other hospital departments as required.
