Job description

Job responsibilities

Job overview

The diabetes and endocrine service allows adequate exposure to subspecialist interests whilst also providing exposure to general medicine. The support of the consultant colleagues in the department allows rotation between inpatient specialist diabetes and diabetes foot care services and general medicine cover. The general medicine cover is split between colleagues and involves ward rounds of 14 inpatients on our dedicated ward

The post holder will conduct ward rounds with the junior doctors, ensuring all patients are seen daily. This will develop leadership and supervision skills as the Registrar is a key link in co-ordinating junior doctor leave on the ward and ensuring that all patients are safely reviewed when shortfalls occur.

The post-holder will help develop the diabetes and endocrinology services appropriate to the need of the population, in partnership with the other colleagues in the department and with support from Clinical Director. We run a busy diabetes service and see approximately 120 new patients and 300 follow-up patients per year. Our endocrinology service sees approximately 320 new patients and 800 follow-up patients per year. The post will have mentorship support from the Diabetes and Endocrinology consultant lead. The Trust is seeking a clinician committed to service development and interested in taking on active leadership roles within the department.

A key role of the post will be in the provision of new sub-specialist multidisciplinary diabetes foot care clinics. The post will also help develop the endocrine service and support in patient diabetes and endocrine care. We are happy to support the development of subspecialist interests, including development of an insulin pump service.

The post holder will also work closely with the Diabetes Inpatient Specialist Nurse team. The inpatient service is designed to deliver improved measurable diabetes outcomes- notably enhanced patient safety, reduced admissions, readmissions and reductions in length of stay.

The Registrars play a key role in ensuring medical team input into the Red2Green meeting, which takes place twice daily on each ward. This is a ward based

Mini MDT in which the treatment and discharge plan for each patient on the ward is reviewed with a view to eliminating bottlenecks and non-treatment related delays.

The post holder will assist in teaching and supervising the FY2 and FY1 and CMT1/2 in practical procedures and in their clinical learning.

This new post has arisen in order to maintain high quality local services in diabetes and endocrinology and to deliver commissioned clinical activities in line with NICE guidelines and the National Service Framework for Diabetes. Our NHS trust has been successful in introducing new diabetes services as part of the NHS transformation project. This includes a new multidisciplinary diabetes foot care service and a diabetes in-patient service. The successful applicant will assist with these new services. This will improve outcomes and patient experience for patients with diabetes treated at our hospital trust.

Main duties of the job

  • To co-ordinate high quality person-centred care of the patients on the ward, ensuring that all patients admitted have a diagnosis and treatment plan, and receive a daily medical review. To participate in outpatient clinics where required.
  • To provide senior medical leadership on the ward by supporting and supervising more junior staff and delegating duties appropriately.
  • To ensure the appropriate discharge of patients, including providing timely paperwork to the General Practitioner ensuring a continuation of care following discharge
  • To actively participate in any appropriate steering or project groups.
  • To build and maintain excellent relationships with all members of the multidisciplinary team, working with them and communicating effectively to deliver high quality patient care.
  • To engage fully in development of services, driving forward innovations to meet the anticipated future challenges of the ageing population.
  • The post holder will be a member of the Medical Healthcare Group and will be expected to participate in the activities of the directorate as indicated.

This job description may be subject to change according to the varying needs of the service. Such changes will be made after discussion between the post holder and the manager. All duties must be carried out under supervision or within Trust policy and procedure.
