The Elderly Medicine department has seen significant financial investment from the Trust in the last 2 years in way of a purpose built frailty unit and expansion of the Elderly Medicine multidisciplinary team to enable delivery of the best possible care to our frail, older population. We are looking for a successful applicant to join us on our journey to further develop the services already provided and introduce a passion for specialist services.

Main duties of the job
To provide high quality inpatient and outpatient services which are coordinated and person-centred to the elderly population served by PAH. The Geriatric team are undergoing an exciting development of elective and specialist services provided including the introduction of a cancer frailty clinic to support the colorectal cancer pathway.
To lead and motivate the multidisciplinary team within his/her ward area in order to deliver effective patient-focused interventions and timely discharge planning.
To create and maintain excellent working relationships with their own team, management team and external agencies such as community providers and CCGs. This includes effective communication in line with the Trust values
To engage fully in development of services to meet the anticipated future challenges of the ageing population.
To lead on and drive forward innovations within service area.
To support consultant colleagues in the department in the delivery of their services where necessary

Services include our front-door (same-day-emergency-care) service, short stay unit as well as ortho-geriatric ward services, community clinics, Parkinsons Disease clinics, cancer frailty clinic and general clinics, all designed to support our patients in their later years by meeting their changing needs seamlessly and avoiding crisis moments..
The job plan will include a minimum of 8.5 PA direct clinical care as well as supporting professional activities. Direct clinical care will include ward rounds and outpatient clinics based at either at Princess Alexandra Hospital, Harlow or Herts and Essex, Bishops Stortford.
The successful applicant will participate in the Frailty weekend on-call rota with the option for additional PAs or the job plan can be suitably modified for a candidate wishing to work a total of 10 PAs.
