You willlead our clinical pharmacy service within the respiratory department and on the respiratory ward.
You will be working closely with specialist nurses and consultantsmanaging high risk drugsand developing pathways and implementing shared care pathways as well as treatment protocols and guidelines.
You will also work as part of a respiratory ward based multidisciplinary teammanaging medicine related issues and improve patient flow by facilitating discharge.
Main duties of the job
Main duties of the job
- To lead and manage clinical pharmacy services and high cost/homecare medication within the respiratory department, in conjunction with Lead Pharmacist for Medicine
- To provide a high-quality clinical pharmacy service to the respiratory ward including supervision and training of junior pharmacy staff
- To support patient flow within hospital by working with the respiratory team to manage discharge of complex respiratory patient including prescribing of TTA medicines.
- To lead work in managing the high-risk drugs within respiratory medicine by setting up monitoring systems and implementing shared care.
- To support and manage the validation of TB prescriptions.
- To lead, manage and develop medicines management initiatives to respiratory patients in the trust.
- To develop protocols, pathways and shared cared guidelines within the respiratory department
- To develop protocols to maximise gain share from future biosimilar drugs.
- To provide evaluated pharmaceutical and financial information in accordance with the needs of the division, including horizon scanning, prescribing practices and cost saving opportunities
- To train, and develop all staff working within the team

Service Provision
General
- Act as a specialist role model and expert in the medicine management within respiratory department.
- Ensure a high level of clinical pharmacy input to the care of patients
- Providing interface and discharge liaison and support.
- Support patient flow within hospital, by working with discharge teams supporting with TTA writing and transcription
- To ensure the smooth running, day to day management of the pharmacy clinical service on the respiratory ward,
- To ensure the safe, clinically effective and cost-efficient use of drugs
- To meet any medication-related needs of respiratory patients,
- To support clinics and ward in the management of patients with severe asthma and other complex respiratory conditions
High Cost Medicines
- To lead on the repatriation of respiratory patients receiving high cost drugs from tertiary centres to ensure the smooth transfer of care and monitoring of patients.
- To work with the specialist nurses to keep a database of all transfers,
- To manage the homecare pharmacy service to the respiratory patients,
- To improve mechanisms within the respiratory department to minimise waste with Homecare biologics.
- To optimise funding applications for biological therapies that are used outside of national guidelines
- Undertakes research into homecare medicines management services,
- Develop and manage biologic review meetings to discuss all biologic initiations and yearly reviews
- To manage non-biologic patients through dose adjustment and drug switching
