I am enormously proud of our Children and Young People's services at The Princess Alexandra Hospital as the Head of Nursing. I am looking to build my senior management team and as such I am excited to recruit to the post of Children and Young People's mental health liaison nurse, to join me and my existing team.

I am looking for an exceptional, inspirational, dynamic and highly experienced senior nursewhose role is key to support the development of our mental health services. This will include working with the senior team in supporting and developing the workforce to meet our CQC legislative requirements. We are an innovative paediatric service with an ambitious future.

Main duties of the job

To provide visible, credible professional clinical leadership, supporting the clinical management of children with mental health conditions and eating disorders who have been admitted on to the inpatient ward.

Job description

Job responsibilities

  • To communicate complex and sensitive information and advice to child or young person and their families, carers and professionals, including information about diagnosis and prognosis, to find ways of enabling the information to be understood, providing empathy and reassurance.
  • To be responsible for the assessment, planning, implementing and evaluating of specialist care, including supporting and evaluating the work of paediatric and agency staff
  • You will be expected to actively involve service users in decisions about the service and support they receive, to be aware of and inform service users about participation initiatives, and actively support the development of such initiatives to ensure the voice of service users is at the heart of service design, development and delivery.
  • To develop and facilitate, with other members of the service, clinical groups, training sessions and workshops as appropriate.
  • To work on the ward as a senior member of the team when no children or young people with mental health disorders are admitted to the unit
  • Improve the quality of care for children and young people experiencing mental problems whilst they are being cared for at their Trust

  • Reduce emergency mental health readmissions and ED re-attendances for young people with mental health problems by:
        • Securing onward referral to community mental health services where appropriate; and Engaging with local CAMHS and eating disorder services to create supportive care plans for those with recurring attendances
  • To support the reduction of the length of stay for patients through interagency assessment and planning from point of attendance. Improve the quality of care for children and young people experiencing mental problems whilst they are being cared for at their Trust
  • To ensure the nursing care delivered to the children and young people with mental and physical health needs, is of a high quality, research based, and is patient and family focused.
  • To foster a close working relationship with clinicians and Allied Health Care Professionals to ensure a multidisciplinary and interagency approach to patient care.
  • To assist with the management and de-escalation of the behaviour of children and young people who at times may be physically and verbally aggressive towards other members of the family and staff.
  • To facilitate comprehensive risk assessments and management plans, involving the service user, carer, and any other individuals relevant to the care of the service user, and to ensure that this is well documented for the duration of their inpatient stay.
  • To receive regular professional supervision.
  • To maintain knowledge of relevant legislation in relation to children and adolescents including safeguarding children and young people, child protection procedures and policies.
  • To update training regularly on safeguarding and other relevant mandatory topics.

Clinical

The post holder will:

  • To support staff to carry out in house risk assessments to promote the safety of the young person in the department.
  • Support clinicians with capacity assessments in patients with complex diagnosis or requirements.
  • Facilitate the development of a working diagnosis in conjunction with medical/psychiatric staff through the provision of reports, assessments and observations.
  • Advise that care plans accurately reflect the patients mental and physical health needs
  • Ensure staff are aware of the process of medical recording and uploading onto their respective clinical system (and shared with referrer for those attending A&E) within 24 hours;
  • Be responsible for acting as the link between acute care, eating disorder and mental health services
  • Be fully engaged in the early discharge planning for patients;
  • Monitor the referral of patients to other mental health services and eating disorder services.
  • Engage with acute staff over physical health conditions requirements or ongoing concerns.
  • Provide ongoing monitoring and advice on the management of mental health problems and psychiatric medication affecting physical healthcare while the patient remains an inpatient under CYP services.
  • Mental health care plans will be integrated into summary discharge plans where appropriate (when on-going mental health issues have been identified).
  • For urgent referrals for people experiencing an acute crisis in their mental health the following care and treatment interventions will be provided:
  • The post holder will be part of the crisis pathway as trusted assessors, to enable a streamlined referral pathway for those presenting at high and immediate risk to self or others and avoiding the need for duplicate assessment processes where this is clinically appropriate.
  • Provide information and advice on risk management;
  • Raise awareness of the importance of mental health;
  • Promote equality to reduce stigma of access to appropriate services.
  • Be an advocate for children and young people with eating disorders and or mental health issues.
