Junior Clinical Fellow in Paediatric Emergency Medicine

Job summary

5 Junior Clinical Fellow posts (ST1-3 level) in the Paediatric Emergency Department for 6-12 months starting September 2026.

We are looking for doctors who have an interest in either Paediatrics or Emergency Medicine, and who want to gain experience in the assessment and management of acutely unwell and injured children. We have a very strong track record of fellows obtaining training posts in their chosen specialty. Candidates are expected to complete QI projects and to engage with the tailored education program, with dedicated education shifts.

St Mary's Hospital is a Major Trauma Centre in London, and has a range of paediatric medical and surgical specialities, including PID, Haematology, Neurology, Allergy, Renal and PICU. It is one of the key trusts in the North London Paediatric Specialist Network, and works closely with West London Children's Hospital.

It has an active and internationally respected academic and research unit, with current recruitment to several national significant research projects.

The department is staffed by dedicated paediatric nurses and led by a team of PEM consultants. Trainees come from both paediatric and emergency medicine backgrounds on a 2-tier rota, and a senior decision maker in the department 24/7.

Candidates MUST have completed their foundation training, and worked in the UK NHS for at least 12 months.

Main duties of the job

The posts are based entirely within the Paediatric Emergency Department. Doctors will provide the initial assessment and management of unwell and injured children who present to the Emergency Department, or who are referred by a primary care team. The post will also include exposure to critically ill children and emergency ambulance conveyances. Post-holders will hone both their clinical reasoning skills and their practical procedures, as well as their skills in verbal and written communication and patient notes.

They will be supported by registrars and consultants based in the PED, and the nursing and therapy staff. The post includes out of hours shifts, including late and night shifts, on a 9 person rota. There are medical and surgical specialties on site (including PICU), as well as the major trauma service (24/7 consultant led).

There is a 4 bedded, co-located Clinical Decision Unit that helps flow through the department, and the team manage these patients as well as those in the main department.

There is dedicated education shifts within the rota, and a weekly teaching rota including in situ simulation, journal club, lightning learning, and formal teaching sessions.

As a large teaching hospital, there are regular placements for medical students, and frequent opportunities to teach.

About us

The department is staffed with a dedicated PED team.

There are 11 consultants (8 WTE) who provide shop-floor cover until 10pm seven days a week, with on-call support at all other times. They all have subspecialty training in PEM (RCPCH or RCEM), and have interests and expertise in a wide range of areas, including research, POCUS, education, trauma, informatics and health policy.

There are 6 registrars and 9 junior fellows who make up the medical team, a mix of training posts and fellow posts, and including both paediatric and emergency medicine doctors. To mirror the pattern of attendances, the rota has both daytime and late shifts, as well as night shifts, and includes working 1 in 3 weekends.

There is a full cohort of paediatric nursing staff, led by the PED matron and the Band 7 team, ensuring senior nursing support on each shift. The majority are paediatric ED nurses, with both adult ED and general paediatric nurses on rotation through the department. There is a dedicated play specialist for the PED. There is also strong support from the paediatric safeguarding team and from Red Thread, a charity helping young people involved in violence and gangs.

The department had a long tradition of research and innovation, led by Dr Ruud Nijman. It is a leading member of PERUKI (Paediatric Emergency Research network for UK & Ireland) and has close ties with the Centre for Paediatrics and Child Health, Imperial College London.
