Job summary
An exciting permanent band 7 opportunity has arisen within a dynamic multidisciplinary integrated community respiratory service.
We are looking for an enthusiastic candidate with a significant experience in respiratory physiotherapy or nursing to join the service this. Previous experience of working in the community is desired but not essential. This role would suit either a current 7 or band 6 looking to develop and gain experience.
The current responsibilities of a band 7 includes but not limited to triage and management of community acute COPD exacerbation, working closely with virtual ward, admission avoidance. Escalating when necessary and ensuring seamless continuity of care post hospital discharge, clinical review of patient self-management, optimisation and education in COPD and Asthma.
There may also be active involvement in pulmonary rehabilitation, home oxygen, physiotherapy services including airway clearance, dysfunctional breathing and chronic dyspnoea management and addressing wider determinants of health.
The role is currently mostly clinical with cross-site working, time spent patient facing in the community or situated in office with opportunities to up-skill, partake in post graduate short courses and be involved quality improvement.

Main duties of the job
- To facilitate and manage the provision of high quality physiotherapy/nursing care to all adult respiratory patients alongside the consultants, allied health professionals and nurses in respiratory medicine in a variety of settings to promote respiratory health within Imperial College London NHS Trust and the surrounding community.
- The respiratory practitioner will provide support and advice to doctors, nursing staff, allied health care specialists, patients and their families/carers, known to the community service, and ensure smooth transitions of care into and within the community setting.
- The respiratory practitioner will have the opportunity to rotate through different specialisms and settings within the Integrated respiratory service.
