Job summary
An exciting opportunity has arisen for an experienced cardiac nurse to join our Cardiac Health and Rehabilitation programme based in Hanwell Health Center as part of the Imperial Healthcare Cardiovascular Health Services.
The Service provides care to patients who have been discharged from hospital and are attending the cardiac health and rehabilitation programme.
You will be a Registered Nurse with post registration experience including a least 2 years cardiac experience at senior level preferably in both cardiology and cardiothoracics.
You will also have experience of working within a cardiac rehabilitation setting or be very enthusiastic about this area of work. You will be self motivated, possess good organisational skills, and be able to work alone and as part of a team.
In conjunction with the Lead Nurse and the multi-skilled cardiac health and rehabilitation team you will be responsible for the continuing development of the service, as well as assessing and managing a caseload of patients and participating in the Cardiac classes. The post holder will also act as a resource to other professionals on cardiovascular disease and associated treatments and issues.
You will be taking part in ongoing audit and research activities to maintain quality care provision. Excellent communication skills are essential, as you will need to be committed to sharing of good practice, development of robust professional networks and support of colleagues.
Main duties of the job
- Support the cardiac rehab team day to day to ensure the programme runs effectively and efficiently
- Provide in-patient services when required to identify and recruit patients and their families.
- Assess the risk factors and needs of patients with cardiovascular disease and plan an agreed package of care with the patient
- In line with the cardiac rehab protocol undertake full nursing assessment, implementation and evaluation of patient, and families/carers. to provide specific expert nursing care, advice, support and counselling where necessary.
- To communicate complex information following assessments and analyse information which is sensitive to patients/carers and health professionals.
- Support the supervised exercise sessions of the cardiac rehab programme and deliver health education sessions as required.
