Job summary
This is an exciting opportunity for an experienced renal nurse to play a central role in pioneering a new and innovative Accelerated Kidney Care Pathway. As part of our Advanced Kidney Care team, you will help shape a pilot service designed to support people living with chronic kidney disease who present late to renal services and require timely preparation for renal replacement therapy (RRT).
You will work autonomously, using your specialist knowledge to deliver high quality, person centred education and support to patients and their families. This role offers the chance to influence real service improvement, improve accessibility to home therapies and transplantation, and contribute directly to the development and evaluation of a new model of care.
You will be supported by the Chronic Kidney Disease (CKD) Lead Clinical Nurse Specialist and will be part of a compassionate, forward thinking multidisciplinary team committed to improving early education and outcomes in kidney care.
A strong background in peritoneal dialysis is desirable but not essential, as full training can be provided. Secondment opportunities may be considered for internal candidates.
Main duties of the job
- Providing specialist 1:1 education on RRT options, including home therapies and transplantation.
- Running and contributing to outpatient clinics, ensuring timely follow-up and continuity of care.
- Working autonomously while collaborating closely with the wider multidisciplinary team to ensure a seamless patient journey.
- Assessing patients' clinical and psychosocial needs and developing personalised care plans.
- Supporting and guiding patients who are newly referred and require prompt preparation for RRT.
- Contributing to service development activities, including pathway evaluation and strategic improvement meetings.
- Maintaining high standards of clinical practice, ensuring safe, evidence based care aligned with Trust policies and governance frameworks.
