An exciting opportunity to enhance your career is available - join our Front Door Therapy Team based within the Older Person Assessment and Liaison (OPAL) Team at Charing Cross Hospital. Our Front Door Therapy Team/OPAL provides opportunity for highly motivated therapists to expand their skills in admission avoidance, complex discharge planning, and frailty. We pride ourselves in providing a high quality of care for our patients utilising the Comprehensive Geriatric Assessment (CGA) and working along-side our Frailty Clinical Nurse Specialists, Geriatricians, Doctors and wider multidisciplinary team.

This is a role which sees high quality, positive impact on its service users, reducing length of stay and encouraging patient involvement all stages of decision making. You will be primarily based within the OPAL team working on the acute medical wards. A high level of competence with respiratory patients is necessary for this role, as the post holder will oversee the respiratory caseload on the acute medical wards and provide management of acutely unwell and deteriorating patients.

The post holder will also be expected to work and provide leadership across the wider Front Door Therapy Team, which includes occasional input into the Emergency Department. A high level of confidence in decision making is required for this post holder to formulate and guide patient pathways, whether that be for discharge back into the community or onto our downstream hospital wards.

Main duties of the job

  • Our OPAL/Front Door Therapy Service operates from 8am-4pm or 8:30am-4:30pm Monday to Friday, depending on shift pattern. This role may also have occasional varied shift patterns of 7.5 hours between the hours of 8am-6pm, if supporting the wider Front Door Therapy Team. You will also be a part of the weekend service in OPAL and ED, prioritising patients medically optimised for discharge, where you will then be allocated rest days the following week.
  • The post holder will be responsible for writing rosters for week day and weekend service provision, in collaboration with our other Band 7 therapists and the Clinical Lead Therapist.
  • As a Band 7 Specialist Physiotherapist, Acute & Emergency Medicine, the post holder will be responsible for performing advanced assessment of patients with diverse presentations and complex medical, physical and psychological conditions, to develop and deliver an individualised management/treatment programme, relevant to specialism.
  • Following assessment the post holder will be responsible for making therapeutic and/or clinical diagnoses and treating their own caseloadof specialised and complex patients/clients and maintaining associated records.
  • The post holder may lead a team and be responsible for providing an advanced standard of care to patients at Imperial College NHS Healthcare Trust, which covers a multi-cultural and diverse population
  • The post holder may be rotational and will be expected to work across all sites of the ICHNT.
