Job summary
This is a fantastic opportunity to work in a multidisciplinary nutrition team. The post-holder will have experience of working surgery and it is useful to have some experience of gastroenterology and critical care as allied specialties though this is not essential.
You will:
- Be involved in the day to day screening and clinical monitoring of patients on parenteral nutrition (PN) and attend multidisciplinary team meetings.
- Be required to keep up to date on nutrition, join specialist interest groups such as UKCPA and BAPEN.
- Have a keen interest and experience in adult Surgery. Your role will involve supervising, training and mentoring junior pharmacy staff.
- Work with the Nutrition Steering group and multidisciplinary team to review and rationalise the choice of PN, develop and maintain relevant procedures, guidelines and electronic prescribing affecting Nutrition and Surgery.
- Provide medicines expenditure surveillance in parenteral nutrition and wider use of medicines across Surgery.
- Provide specialist pharmacist support in surgical areas trust-wide.
You will be mainly based on the Charing Cross hospital site clinically, as well as cross site to undertake Surgery Pharmacy team-based activities including attendance at meetings, nutrition MDT meetings/ward rounds, pharmacy directorate level and educational activities as per service requirements.
Main duties of the job
- To support the Pharmacy Department and Trust in leading, managing and developing a high-quality patient focused pharmacy service to those patients requiring parenteral nutrition (PN). This role is for adult patients, the majority of whom will be within the surgical and intensive care directorates, however some patients will sit in other divisions/directorates.
- To order (PN) following clinical review of nutritional needs, undertaking necessary monitoring, liaise with the multidisciplinary team on the nutritional management of patients and manage the operational ordering of PN.
- Be a proactive member of the nutrition team undertaking in multidisciplinary nutrition meetings and supporting the evidence based clinical use of parenteral nutrition.
- To facilitate the clinical and cost-effective use of parenteral nutrition ensuring that pharmaceutical service use and provision is safe and effective.
- To provide clinical pharmacy services, department and ward based, in line with Trust procedures and national legislation.
- To lead, deliver, develop and evaluate clinical pharmacy services in patients receiving parenteral nutrition and supportive nutritional care.
