Aortic Dissection Advanced Nurse Practitioner

Job summary

There are 20,000 people living with aortic dissection in the UK. We are excited to announce that The Aortic Dissection Charitable Trust have awarded us funding to recruit a Band 7 Aortic Dissection Advanced Nurse Practitioner, 17.5 hours per week for an initial period of 3 years as a recognised Aortic Dissection Centre. During this 3-year period the Trust will be working towards converting this to a permanent role.

The post holder will be in a unique position to be an integral part of the team providing lifelong, complex care for acute and chronic aortic dissection patients. This role emerged from the NHS' need to care for a growing population of patients with this life-threatening condition.

As first point of contact you will provide consistent support, manage patients requiring long-term surveillance and help navigate patients through their complex care needs via face-to-face reviews, email contact or via telephone consultations/conversations.

You will be a pivotal member of the multi-disciplinary team, shaping the service that is provided through collaboration, service co-ordination, education, audit, research and patient support. You will work closely with The Aortic Dissection Charitable Trust and play a key role in the implementation of The NHS England Acute Aortic Dissection Toolkit.

You will be supported by the Lead Cardiothoracic Advanced Nurse Practitioner, the Consultant team and The Aortic Dissection Charitable Trust.

Main duties of the job

  • The TADCT advanced nurse practitioner is an experienced registered healthcare professional with specialist expertise in the care of patients with acute and chronic aortic dissection.
  • The aortic dissection advanced nurse practitioner will work within the aortic and cardiovascular surgery multidisciplinary team.
  • This role will enhance the experience and outcomes of patients living with aortic dissection, a condition that requires lifelong follow-up and coordinated care.
  • They will be the first point of contact for patients and families. The post holder will contribute to the implementation of the National Aortic Dissection Toolkit to ensure safe, efficient and patient-centred care is delivered.
  • The post holder will be supported by the Cardiothoracic Matron/Lead for Advanced Practice, the Aorto-Vascular Consultant Team, the Aortic Fellow and the Aortic Dissection Charitable Trust (TADCT).
  • Imperial Healthcare has made significant investment in developing new ways of working for senior cardiac nurses and now has Advanced Nurse Practitioners within the cardiac directorate.
  • This post holder will join an existing team of established practitioners and will work across directorate and professional boundaries, encouraging inter-disciplinary working for the benefit of improved patient outcomes and efficient use of resources.

  • Recent & Significant post registration nursing experience within the speciality.
