Op RESTORE Referral Coordinator

Job summary

The role of Op RESTORE Referral Coordinator provides specific support to veterans ofthe Armed Forces who have service-related physical health needs.

This role is part of Op RESTORE which is a national service to provide support for veterans with service-related physical health issues. The service provides navigation to the veteran and clinical signposting through NHS and veteran services.

The successful candidate will be linking with GP practices, NHS Trusts and social prescribers to ensure clinical information is available regarding veteran's service-related physical health needs and interrogating the clinical information to develop any onward clinical plan.

In conjunction with the Pathway Navigators, a holistic approach is applied to every veteran referred into the service to ensure their health and wellbeing needs are being supported.

The successful candidate can be based anywhere in England - this is a home-working role with travel expected every few months to London or other parts of England so the candidate will have adequate provision at home for an office to undertake confidential calls.

Main duties of the job

  • Provide clinical interrogation of patient records to determine next steps for onward care.
  • Liaise with military charities, NHS England, NHS Trusts to support ongoing treatment for veterans.
  • Be proactive in raising awareness of the service across England
  • Support areas for national service improvement to enhance the Op RESTORE service.
  • Identify and then follow the patient's pathway linking in with social prescribers, GP practices and non- statutory provision and ensure they have knowledge and support in accessing appointments where necessary.
  • Ensure patient has the necessary knowledge and support to access healthcare, further appointments and holistic support where necessary
