Senior clinical fellow in Neurology

Job summary

This post is to cover a vacancy within the department due other members of the resident neurology team working less than full time. The post would be ideal for a motivated, enthusiastic candidate, wanting to gain experience in Neurology in a regional neuroscience centre.

The Neurology Department provides comprehensive adult neurological care for both emergency and elective work. This includes an inpatient Neurology ward (Charing Cross), Same Day Emergency Care (SDEC), a planned Neurological Treatment Unit (NTU), an Acute Neurology service, a wide range of sub-speciality outpatient clinics and a number of Multi-disciplinary team meetings.

This a large department with over 40 Consultant Neurologists linked to Imperial (based at Charing Cross, St. Mary's or Hammersmith Hospitals), many with clinical duties at allied District General Hospitals and/or research commitments within academic departments at Imperial College.

The Neurology department is complemented by the Hyperacute Stroke Unit, Neurosurgery Unit, Neuroradiology Department (with access to CT, MRI, Nuclear Medicine and interventional neuroradiology) and Neurophysiology Department.

The postholder will be expected to participate in the oncall rota.

Main duties of the job

The post will be full time for twelve months. As Imperial College Healthcare NHS Trust has Services across multiple sites, the post-holders may be required to work at a differing site on certain days of the week. The post-holders will be expected to:

Contribute to the care of neurology inpatients, and the acute neurology liaison service.

Participate in the on-call rota, which includes neurology and resident stroke thrombolysis/thrombectomy duties.

Attend/deliver sub-specialty outpatient clinics.

Attend subspecialty multidisciplinary team meetings and neuroradiology meetings.

Assist with triaging of referrals into subspecialty clinics.

Attend the weekly clinical and academic meetings.

Participate in audit, quality improvement projects and clinical research where applicable.

Involvement in teaching of resident doctors and medical students.

Job responsibilities

The main responsibilities of the post include:

1) Provide high quality care to neurology patients

The post will involve care of neurology inpatients, acute neurology liaison service, outpatient clinics general and subspecialty - and weekly academic and neuro-radiology meetings. The post holders will be expected to participate in the on-call rota, which includes neurology and resident stroke thrombolysis/thrombectomy duties.

The post holder must be medically qualified and maintain GMC specialist registration

  • To develop and maintain the competencies required to carry out the duties required of the post.
  • To ensure prompt attendance at agreed direct clinical care Programmed Activities.
  • To ensure patients are involved in decisions about their care and to respond to their views.
  • The Trust supports the requirements of CPD as laid down by the Royal College of Physicians and is committed to providing time and financial support for these activities.

2) Research, teaching and training

Where possible to collaborate with academic and clinical colleagues to enhance the Trusts translational research portfolio, at all times meeting the full requirements of research governance.

To provide high quality teaching to medical undergraduates and members of other health care professions as required by clinical director.

3) Performance management

To work with medical, nursing and managerial colleagues to ensure high performance in the following areas:

Clinical efficiency e.g. LOS reductions, reducing DNA rates.

Quality of outcomes e.g. infection control targets, reducing re-admission rates

Financial management e.g. identification, implementation and achievement of cost improvement programmes and participating in efforts to ensure services are provided cost effectively e.g. managing locum agency spend, monitoring and managing the drug budget to target, ensuring accuracy of clinical data for the team

Operational efficiency e.g. day-case rates, waiting list activity and demand management.

4) Governance

To review clinical outcomes in designated area using external benchmarking data where appropriate, to identify and advise variances to the divisional director or clinical director.

To participate in clinical audit, incident reporting and analysis and to ensure resulting actions are implemented.

To work closely with the directorate, patient and public involvement panels in relation to clinical and services developments as delegated by the divisional director/clinical director

To participate in ensuring NICE requirements are reviewed and implemented and monitored in the speciality areas.

  • MBBS or equivalent medical qualification
  • Appropriate Royal College Membership (MRCP) or equivalent
  • Full registration with the GMC with a licence to practice

  • Evidence of experience in a range of acute medical specialties
  • Neurology SpR level experience
  • Knowledge of the NHS models of Acute Neurology and SDEC

  • Appropriate knowledge base and ability to apply sound clinical judgement to problems
  • Awareness of the basics of managing neurological disease
  • Able to demonstrate proficiency in a range of medical procedures as an indication of manual dexterity and hand-eye coordination
  • Evidence of competence in management of medical emergencies and in-patients through continuous work-based assessments, portfolio evidence, including log book documentation if applicable
