We are delighted to offer an outstanding opportunity for a highly skilled and experienced practitioner to join our Integrated Discharge Service as a clinical leader in complex discharges. In this pivotal role, you will draw on your deep clinical expertise to assess, manage, and coordinate the discharge of patients with complex needs, ensuring seamless transitions of care and positive outcomes for service users and their families. Based primarily at the Charing Cross hospital site, you will also play a crucial role in supporting and providing coverage across our other sites as required, demonstrating flexibility and a commitment to integrated care delivery across the organisation.
As the clinical lead for the discharge team, you will be working closely with the Discharge Hub Lead at Charing Crossing to oversee multiple aspects of the discharge team, working closely with multidisciplinary colleagues to facilitate timely and safe discharges. This is a high-profile position that will place you at the forefront of service development and integrated care initiatives, allowing you to contribute meaningfully to the continuous improvement of our discharge services.
This role is ideally suited for individuals who are passionate about integrated care, service transformation, and partnership working. You will gain invaluable experience in strategic planning, leadership, and stakeholder engagement--skills that are essential for those aspiring to senior health and social care roles.
Main duties of the job
- Provide clinical leadership and responsibility for a caseload of patients.
- Provide clinical supervision to discharge team leads.
- Provide operational management of the discharge team and discharge lounge ensuring robust service delivery.
- Deputise for the discharge leadership team.
- Lead on operational relationships with community partners and local authority adult social care teams.
- Use operational and performance data to feed into oversight calls and implement the North West London escalation framework.
- Foster strong relationships with the general and acute adult inpatient multi-disciplinary teams in order to promote acute flow and the reduction of discharge delays.
