Job summary
An exciting opportunity has arisen for an experienced and motivated cardiac nurse to join our dynamic Cardiac Health and Rehabilitation Programme, part of Imperial College Healthcare NHS Trust's Cardiovascular Health Services.
This role spans across St Mary's Hospital, Charing Cross Hospital, and Ealing Community, with occasional cover at Hammersmith Hospital.
Imperial College Healthcare NHS Trust is the UK's first Academic Health Science Centre, offering internationally renowned tertiary cardiology and cardiothoracic services, and a strong foundation in clinical and basic science research.
We are looking for a Registered Nurse with:
- A minimum of 2 years recent cardiac experience, ideally across both cardiology and cardiothoracic services.
- A strong interest in cardiac rehabilitation and secondary prevention.
- Excellent communication and organisational skills.
- The ability to work independently and collaboratively within a multidisciplinary team.

Main duties of the job
As a Cardiac Health & Rehabilitation Nurse, you will:
- Assess and manage a caseload of outpatients, including those with cardiac disease, heart failure, and ICDs.
- Deliver cardiac rehabilitation classes and health education talks to patients.
- Act as a clinical resource for colleagues on cardiovascular disease and its management.
- Contribute to ongoing audit and research to ensure high-quality care delivery.
- You may also support patients during the inpatient phase and post-discharge, ensuring continuity of care across the rehabilitation pathway.
