Job summary

Imperial College NHS Trust offers an exciting opportunity for a 12-month fixed term contract in a rehabilitation coordinator role. The postholder will work alongside a substantive rehabilitation coordinator to support patients, families and caregivers following traumatic injuries including brain, spinal and polytrauma. Applications are invited from registered Occupational Therapists, Physiotherapists, Speech and Language Therapists or other Allied Health professionals with a strong clinical background in neurological/trauma rehabilitation from a Major Trauma setting.

The Major Trauma and acute neurosurgery services at St Mary's hospital are based on a 19-bedded Major Trauma Ward and other surgical step down wards.

The postholder will work across these areas and multi-disciplinary teams acting as an advocate for the patients and families and navigating them through their rehabilitation pathway. Liaison with the referrers, Consultants and Rehabilitation MDT is an integral part of the job so excellent communication skills are essential. The postholder will provide support for patients and carers with highly complex rehabilitation needs and will require negotiation skills, empathy and the ability to manage complex situations. The postholder will support both adult and paediatric trauma pathways and have opportunities to support patients in their after trauma care.

The Rehabilitation Coordinator role provides staff with clinical support, knowledge and education of complex rehabilitation whilst also having the opportunity to further expand their own knowledge and skills through close working with rehabilitation centres and our Rehabilitation Consultant and Rapid Access to Acute Rehabilitation therapy leads.

The postholder will support patients and other therapists in the North West London Trauma network and act as a point of contact for patients who have transferred out of Imperial College Healthcare NHS Trust. They will also share learning, education and reflections for the network therapists.

Main duties of the job

  • The rehabilitation coordinator works as part of the Major trauma Multi-disciplinary team. The role is to ensure that patients on the major trauma pathway have their rehabilitation needs identified early. The role will deliver and support the coordination of transfer of information between acute and rehabilitation services and ensure that the pathway from the major trauma centre is clearly communicated to the patient and their family. Liaison with the rehabilitation services and teams will be part of the role to support safe onward referrals and flexible working within the rehabilitation providers
  • The band 7 rehabilitation coordinator major trauma is a clinical role with responsibility for the delivery of rehabilitation prescriptions, rehab assessment, treatments and pathway planning for patients following major traumatic injuries including spinal, traumatic brain injury, polytrauma, chest trauma and orthopedics. Support for service development and the training and development of junior staff. We have integrated physiotherapy and occupational therapy teams in major trauma. There will be a role in supporting patients in their aftercare by providing outpatient clinics for certain cohort of patients.
