Job summary
The nationally accredited NWL Inner Boroughs PR Service (under Imperial) has successfully secured funding for a 12 month fixed term Band 7 Pulmonary Rehabilitation Practitioner to lead on addressing service inequity and long term self-management within the PR service.
The member of staff, along with the Band 6 Social Prescriber and Band 5 Exercise Practitioner will help identify and target areas of low referral/uptake, map against deprivation and address any areas of inequality identified. They will support service promotion and awareness in hard to reach areas and work with community teams and the newly recruited social prescriber to improve patient care and support uptake. This will support the service in addressing health inequity and meeting the NHS long term plan. They will also be responsible for providing a post-PR follow up clinic with the social prescriber to help improve patient self-management and activity level, thus reducing the risk of deterioration/healthcare utilisation through improved self-mastery, patient education and wellbeing tools.
Main duties of the job
- To facilitate and manage the provision of high quality physiotherapy/nursing care to all adult respiratory patients alongside the consultants, AHP's and nurses in respiratory medicine in a variety of settings to promote respiratory health within ICHNT and the surrounding community.
- The respiratory practitioner will provide support and advice to doctors, nursing staff, allied health care specialists, patients and their families/carers, known to the community service, and ensure smooth transitions of care into and within the community setting.
- The respiratory practitioner will have the opportunity to rotate through different specialisms and settings within the Integrated respiratory service.
