- Team:Immune Haematology, including adult, paediatric, and transition services
- Network:Part of the UK ITP forum network
- Focus:Development of a North West London clinical network for the management of Immune Thrombocytopenia (ITP)

Main duties of the job
- Patient-Centred Care: Always ensure that the needs of the patient are at the centre of care delivery. Act as a 'key worker' providing specialist nursing advice, care, & support to patients with immune-mediated haematology conditions.
- High-Quality Care: Facilitate high-quality, individualized care to patients, clients & relatives/carers, both inpatients and outpatients, in collaboration with the multi-disciplinary team and in line with local and national targets.
- Day-to-Day Management:Manage the day-to-day care of patients with immune-mediated haematology conditions, with a particular focus on establishing a North West London (NWL) clinical network for the management of Immune Thrombocytopenia.
- Collaboration:Work closely with eight local partner hospitals, serving a catchment population of 1.5 million, linking with Hammersmith Hospital for ITP clinical guidance and advice.
- Strategic Priorities:Support the Trust and NWL network's strategic priorities to achieve excellent patient outcomes and experience through integrated and financially efficient service models, eliminating unwarranted variation of care across the Trust and other NHS NWL partners.
- Patient Experience:Promote a culture that delivers patient-centred care throughout the pathway, contributing to improved patient experience.
- Specialist Resource: Act as a specialist resource for patients and the clinical team by providing comprehensive advice, information, teaching, and training both within ICHT and externally.
