Job summary

An exciting opportunity has arisen within the Integrated Virtual Wards Team for an experienced nurse to contribute to the success and expansion of our Virtual Wards Programme.

This role will be instrumental to identifying patients suitable for admission or discharge onto Virtual Ward pathways, reviewing referrals and safely onboarding these patients.

Delivering digitally enabled, virtual care is a national priority embedded within current health and social care policy. Virtual wards are a safe and efficient alternative to NHS inpatient care. They allow patients, who would otherwise be in hospital, to receive the acute care and treatment they need in the comfort of their own home through the use of digital technology.

This helps to prevent avoidable admissions into and supports early discharge out of hospital, meaningfully improving patient outcomes and experience and hospital flow, whilst also releasing pressure on acute care.

Main duties of the job

  • Lead case finding of patients suitable for discharge from hospital with virtual ward support by working with acute medical and wards teams and using digital solutions.
  • Receive referrals from teams and review and assess their appropriateness for admission to virtual ward pathways.
  • Provide leadership for digital pathways when working with teams from across the trust.
  • Onboard patients on virtual ward pathways.
  • Ensure that all patients are provided with a safe and high quality standard of care.
