Job summary
The Integrated Virtual Wards Team are looking for two enthusiastic and proactive nurses to support the growth and success of our Virtual Wards Programme. These roles will be crucial to the safe admission of patient onto our virtual wards and their monitoring during their stay.
These posts offer great development opportunities through three-month rotations between the following two areas (six months in each):
1) Working across all ICHT sites to support the referral and onboarding of patients onto our Virtual Wards pathways2) Working with our colleagues on the NWL Virtual Hospital based at St. Charles Hospital to provide remote care.
Virtual wards are a safe and efficient alternative to NHS inpatient care. They allow patients, who would otherwise be in hospital, to receive the acute care and treatment they need in the comfort of their own home through using digital technology. This helps prevent avoidable admissions and supports early discharge out of hospital, improving patient outcomes and experience, whilst also reducing pressure on acute care.
Main duties of the job
- Carry out planned care for a group of patients without direct supervision, utilising escalation pathways to ensure care is coordinated into organisations in North West London.
- Ensure the assessment, planning, implementation and evaluation of evidence based, individualised patient care, including supporting case finding, reviewing referrals from teams and onboarding to Virtual Wards pathways.
- Work collaboratively with colleagues from across the organisation and externally in order to meet patient needs.
- Assess the needs of patients, relatives and carers and provide them with information as requested.
- To keep up to date with current developments in nursing and ensure evidence based practice.
- To be aware of and take appropriate action in regard to cardiac arrest, fire and major incident
- Be aware of and adhere to Trust policies and procedures.
