Job summary
An exciting opportunity has arisen for an experienced cardiac nurse to join our Cardiac Health and Rehabilitation Programme in Ealing Community Team as part of the Imperial Healthcare Cardiovascular Health Services.
The Cardiac Health and Rehabilitation Programme provides care to patients during the in-patient phase and to those who have been discharged and are attending the cardiac health and rehabilitation programme. It includes patients with cardiac disease, heart failure as well as those with ICDs.
As a Cardiac Health & Rehabilitation Nurse you will assess and manage a caseload of out-patients and participate in the virtual and face-to-face Cardiac classes. You will act as a resource to other professionals on cardiovascular disease and associated treatments and issues. You will be taking part in ongoing audit and research activities to maintain quality care provision. You may also be required to provide occasional cover for our in-patient service at Hammersmith Hospital.
You will be a Registered Nurse with a minimum of 2 years recent cardiac experience preferably in both Cardiology and Cardiothoracic with great enthusiasm for cardiac rehabilitation.
You will be self motivated, possess good organisational skills, and be able to work alone and as part of a team. Excellent communication skills are essential, as you will need to be committed to sharing of good practice, development of robust professional networks and support of colleagues
Main duties of the job
In this role, you will support the cardiac rehabilitation team to ensure the program operates effectively and efficiently. You will also provide in-patient services when necessary, identifying and recruiting patients and their families.
Key responsibilities include assessing cardiovascular patients' risk factors and needs, planning individualized care packages, and delivering expert nursing care, advice, and support.
You will lead full nursing assessments, implement and evaluate care plans, and communicate complex, sensitive information to patients, families, and healthcare professionals. Additionally, you will facilitate supervised exercise sessions, deliver health education, and coordinate patient care across the treatment pathway to improve outcomes and satisfaction.
The role involves attending multidisciplinary meetings, providing input on physical and psychological care, and assisting with discharge planning. You will also maintain patient records, offer ongoing support to patients and families, and work flexibly across different locations and times to ensure accessibility of the programs.
