Job summary
An exciting opportunity has arisen for a clinical nurse specialist to join the evolving Frailty Team. The Frailty Team currently provide an in-reach service into the Emergency Departments (ED) at both our St Mary's and Charing Cross hospitals, assessing patients using a comprehensive geriatric assessment (CGA) framework, with the aim of admission avoidance. The service is looking to expand its service by providing rapid access clinics for complex case management and admission avoidance.
The successful applicant will have experience working with frail patients, ideally within specific frailty services. The role will involve specialist assessments that encompass both nursing and therapy skills to best meet the needs of this complex cohort of patients. Most importantly, the successful applicant will have a passion for older people's care.
Please note that this position requires the successful applicant to be able to cover both the St Mary's and Charing Cross sites. Currently the service does not operate at weekends and bank holidays. This is subject to change as the service expands.

Main duties of the job
- Working as a clinical nurse specialist facilitate efficient, effective, and co-ordinated assessment and treatment of frail elderly medical patients primarily within the emergency department and other front door pathways including acute assessment areas and rapid access clinics.
- Provide specialist nursing advice to support and facilitate high quality, individualised care to patients/clients and relatives/carers without supervision in collaboration with the multi-disciplinary team in line with local and national targets.
- The post holder will provide specialist nursing care and support and deputise for their line manager in the management of their clinical speciality. This will involve dealing with complex situations and having to take appropriate action in a variety of settings.
- To be flexible in the approach to the clinical area and to provide cover for any area specified acting as professional role model providing direct and indirect clinical care, teaching and research.
- There may be weekend and Bank Holiday cover expected as part of the role. You must be able to work at both the Charing Cross and St Mary's sites according to the needs of the service.
