Job summary
This post is based in our growing department of elderly medicine and frailty specialists. Our department is full of opportunity for a new or established consultant physician to develop their career in general and subspecialties of geriatric medicine. The role will include time with the newly expanded acute frailty service in acute admissions and the Emergency department, supervising a team of doctors and Band 7/8 Frailty practitioners to deliver high quality comprehensive assessment to frail patients at the front door of the hospital. You will also join a large number of physicians and specialties in contributing to the acute medical rota, with periodic acute blocks.
Main duties of the job
We currently seek to recruit two consultant geriatricians in Acute Frailty. You will join our existing team of consultant geriatricians, working on the St Mary's site. As well as expanding and improving our provision of acute frailty services in medical admissions and the emergency department and contributing to our community sessions in West London, the post offers opportunities in undergraduate and postgraduate education, service development, ICT and innovation, quality improvement and management.
The first post is intended as a full time or near-full time post. The second post is a less-than full-time job share with an existing frailty consultant. Between the three posts rotational cover will primarily provide for daily acute frailty ward rounds and new patient reviews in acute medicine and ED CDU, and support for frailty practitioners in ED. The secondary responsibilities (cancelled during cross cover for primary acute frailty cover) will be providing a Rapid Access clinic for ward discharges/community referrals and SDEC transfers, and sharing in the department's multiple community Service MDTs providing geriatric expertise to local nursing homes, GP practices, and Rapid Response / Rehab services.

Medicine for the Elderly (MfE) services have evolved over recent years at St Marys Hospital (SMH) so there is now a multiprofessional Acute Frailty Team and ED liaison service, in addition to the 48 acute MFE beds across 3 wards, outpatients and liaison work with multiple community services in West London. The acute frailty team requires daily consultant input and so in addition to the current job-share arrangement, this new post has been created to allow for year-round provision of acute frailty input on admissions wards and into the Emergency Department, and further to allow the development of Frailty OPRAC (Older Persons Rapid Access Clinic) at the SMH site.
Frailty service development is at the heart of the Trust's planning and operations, and the addition of this post holder will allow not only provide the requisite level of cover to maintain a constant service, but also the capacity to design and pilot improvements within the team. There is an active programme of audit and QIP within and around the department and an active working group with the ED who have an appointed frailty lead. The post holder will be able to participate on local and regional working groups and CQUINs we are currently signed up to the Frailty Assessment CQUIN in ED.
This post holder will work with 2 consultant colleagues in the frailty service and will be primarily based in the acute medical unit, providing acute frailty input to admitted medical patients. Additionally, supervision and collaboration with the emergency department-based Frailty Advanced Practitioners (ACPs) is a key component. There will also be opportunities for involvement in community and virtual ward input, and general medicine admissions / on-call shifts. The post-holder will rotate through cover for the acute admissions area, ED, Rapid Access clinics, and some periods of ad hoc cover of other inpatient services. During each block there will be also opportunities for community involvement including West London GP practice and Nursing home MDTs.
