Job summary
The most recent national lung cancer audit attributed 296 cases to ICHT, the second highest in our region. Our data recording and completeness was the best in the region and our performance measures are also above national standards.
The lung cancer service is supported by 6 CNSs across the sites and an MDT coordinator and data tracker. The team run a diagnostic MDT once a week and a treatment MDT once a week. Both MDTs serve pan-site patients, including patients from the Hammersmith site. A dedicated pulmonary nodule MDT sits within the diagnostic MDT and is similarly supported by the MDT coordinator and the radiology team.
The lung cancer service is supported by 5 full time and 1 part-time CNSs across the sites and an MDT coordinator and data tracker, with a plan to recruit a further lung CNS. There is one cross-site lung cancer MDT and a separate cross--site lung nodule/diagnostic MDT each week.
We have two clinical oncologists and one medical oncologist. We have a four full time Thoracic surgeons, supported by dedicated thoracic surgery clinical nurse specialists. We have an excellent thoracic radiology service, with 5 consultants and full interventional radiology services. We have 3 specialist respiratory histocytopathologists.
ICHT is a specialist centre for a number of other cancer sites and a significant proportion of the MDT and nodule workload is generated by other cancer MDTs and the service has strong links with all of the other cancer MDTs as a result.
Main duties of the job
This is a one-year clinical fellowship post designed to provide the clinical fellow with the unique opportunity to work with the lung cancer clinicians at Imperial. The role will involve supporting the lung cancer screening service through the Screening review meetings (SRM) and the post holder will be responsible for enacting the outcomes from these meetings. The fellow will support the clinical services so that they can understand the clinical pathways and be immersed in the service - they will be expected to attend and support the MDTs - and do clinics, they will be supported by the lung cancer clinicians and work closely with the clinical nurse specialist teams. The post would be well-suited to a respiratory trainee as an out-of-programme experience during specialist training, or as a post-CCT fellowship. The post will provide the fellow with the opportunity to work with the clinical lung cancer team at imperial and gain experience across the whole lung cancer pathway, at the same time as working within the new lung cancer screening service which is a collaboration between Inhealth, The Joint thoracic service (RMH and RBHT) and Imperial. The post will be held within Imperial. There will also be opportunities for the fellow to learn or develop interventional skills in either bronchoscopy/ EBUS or pleural respiratory medicine.

Provide High Quality Care to Patients in Acute & General Medicine and Respiratory medicine

Research

- Clinical efficiency e.g. length of stay reductions, reducing cancelled procedures and DNA rates.
- Quality of outcomes e.g. infection control targets.
- Financial management e.g. identification of cost improvement programmes and participating in efforts to ensure services are provided cost effectively e.g. ensuring accuracy of clinical data for the team.
- Operational efficiency e.g. day-case rates, waiting list activity and demand management.
Governance
The post holder will:
- Review clinical outcomes in designated clinical areas using external benchmarking data where appropriate, to identify and advise variances to the Head of Specialty.
- Participate in clinical audit, incident reporting and analysis and ensure resulting actions are implemented.
- Work closely with the Directorate, Patient and Public Involvement panels in relation to clinical and services developments as delegated by the Divisional management team.
- Ensure clinical guidelines and protocols are adhered to by junior medical staff and updated on a regular basis.
- Keep fully informed about best clinical practice and ensure implications for practice changes are discussed with the Divisional management team.
- Role model good practice for infection control to all members of the multidisciplinary team.
