Job summary
This is a post open to candidates with sub-specialty training, experience and interest in cardiothoracic anaesthesia.
The cardiothoracic unit at the Hammersmith Hospital opened in 2009 following the amalgamation of the units at St Mary's and Hammersmith Hospitals. As well as merging of the cardiac and thoracic surgical workload, a new 16-bedded unit was built for the care of post-operative patients. Since then, there has been a substantial expansion of surgical and cardiology services at the Hammersmith Hospital, with particular growth in structural heart procedures in the cardiac catheter laboratory - TAVI, PFO, Mitraclips - as well as electro-physiology services. Aortic work has also increased with the appointment of two new surgeons, and the Hammersmith Hospital has now joined the regional aortic dissection rota, with the Harefield and Royal Brompton Hospitals. Post-operative intensive care for these complex cardiac cases is delivered by a dedicated team of consultants. In addition there has been a recent expansion in the number of consultant thoracic surgeons who are performing much higher numbers of cancer cases and emergency work. This has vastly increased the need for elective and emergency cardiothoracic anaesthetists.
In addition to cardiothoracic surgery, the Hammersmith Hospital has an expanding major cancer workload including upper GI, hepatobiliary, gynae-oncology and endocrine surgery, and is also a busy kidney and pancreas-kidney transplant centre.
Main duties of the job
This post is intended to support the group of anaesthetic consultants providing cardiothoracic anaesthesia in theatres and the cardiac catheter laboratory. The anaesthetic department is a flexible workforce, with most consultants having a combination of fixed regular lists and flexible sessions, and are to an extent 'annualised' in the delivery of clinical sessions. This arrangement allows us to respond to service requirements efficiently, as well as facilitating CPD, educational and annual leave requirements.
In addition to cardiothoracic cases, the Hammersmith Hospital provides complex cancer surgical services in gynaecology, endocrine, hepatobiliary and upper GI surgery and is a regional and national referral centre for these specialities. The Hammersmith Hospital is also home to a busy renal transplant unit. The successful candidate will have the opportunity to deliver peri-operative care to patients undergoing these complex procedures.
The post includes a 1:8 on call component which will cover cardiothoracic emergencies in theatre and cath lab emergencies, and approximately 50% of the time will include general emergencies as well - at the Hammersmith this is mostly kidney transplants, benign gynaecology and re-operation for surgical complications. When the Hammersmith Hospital is covering aortic dissections, there will be a dedicated on call cardiac anaesthetist for this. There is a resident anaesthetic registrar (ST4-7 level) and CT2/3 covering theatres out of hours.

The on call commitment for the post holder will be a 1:8 non-resident rota. This will either be covering cardiac emergencies, the cardiac catheter laboratory and general emergencies, or aortic dissections alone (when the Hammersmith is covering the regional aortic dissection rota).
SPA time is given to support continuous professional development, teaching, quality improvement and preparation for appraisal.
