An exciting opportunity has arisen for an experienced senior nurse to join our stroke service as a Band 7 Clinical Nurse Specialist (CNS).
The Lead Stroke CNS role is a vital part of the MDT approach to managing and supporting patients presenting with acute stroke symptoms, as well as the chronic and lasting effects of stroke. The CNS is also involved in the thrombolysis/thrombectomy pathway and is a leader in providing secondary prevention education to patients and their families. You will also support the TIA and Apoplex clinics, helping to identify patients with AF and other risk factors, counselling patients following investigations.
In this role, you will contribute to the provision of specialist stroke nursing, practicing within the MDT, providing nursing advice to support and facilitate high quality, individualised care to patients with a diagnosis of a stroke/TIA and relatives/carers. You will also be responsible for highly-accurate data entry and quality monitoring, in line with local and national targets.
This role will allow you to develop wider specialist knowledge and skills working in close collaboration with the other Stroke specialist practitioners within the team.
You will act as an expert resource for other health professionals in relation to stroke nursing and will take a lead role in the holistic assessment of patients.
Main duties of the job
Ensure a seamless service through collaboration with the multi-professional teams across Imperial College Healthcare NHS Trust and the community providing specialised advice and support to patients, families and health professionals through the use of evidence based knowledge.
Develop patient and service driven initiatives and strive to continually improve the stroke service and provide high quality care and expert advice for stroke patients and their families.
Attend thrombolysis and thrombectomy calls when required.
Managed a caseload of patients through the TIA clinic as well as monitoring the progress of the Apoplex programme.
Ensure national standards are adhered to through collaboration with the multidisciplinary team, managers and stroke consultants and raise the profile of nursing and research.
Utilise information technology in order to collate accurate and timely information
