Job summary
An exciting opportunity has arisen for a highly motivated registered adult nurse to join our Imperial College Healthcare NHS Trust based at the Charing Cross Hospital site as a Band 7 Clinical Practice Educator (CPE) with Specialist Neuro Rehabilitation and the wider stroke and neuroscience directorate. The Trust has been successful in being awarded NHSE recurrent funding to provide rapid access acute rehabilitation services (RAAR). These beds will be provided on the Charing Cross Hospital site. This service development will improve the existing patient pathway and experiences, with easier access to rehabilitation beds and an overall reduction in LOS of patients requiring pathway 2, bedded rehabilitation. The RAAR service will also include support for in-reaching and assessing patients across the Trust and outreaching to support transfer of care to the most appropriate onward community services across the NWL sector.
The purpose of this new CPE role is to enable support across the in reach and outreach work load of the service by upskilling nursing teams across the stroke and neurosciences directorate at both Charing Cross and St Mary's sites to provide the high level intensive early rehabilitation that patients identified for pathway 2 bedded rehabilitation.
