Neonatal Critical Care Community Outreach Nurse

Job summary
Our neonatal service is the largest neonatal intensive care unit (NICU) service within North West London and provides care to high risk, critically ill neonates, and integrating the family into care is one of our guiding principles. This supports parents in becoming partners in care, participating in providing active care for their baby with the help of competency based training, support and education.
As a QIS, qualified in specialty nurse you would join the Neonatal Outreach team working across both our hospital sites and in the community. We are looking for a skilled nurse to join the Outreach team to develop your skills in assessing and planning discharges. Our family centred approach supports early discharge for all babies being discharged from our Neonatal units including those with complex needs such as home oxygen and Naso-gastric tube feeding.
In addition, we can support you through MSc/post graduate pathways and advanced leadership training with coaching to continue your development as a leader within your chosen specialty.

Main duties of the job
- The post holder is responsible for the assessment of care needs, the development, implementation and evaluation of programmes of care without supervision in both the hospital and the community setting
- The post holder will work as part of a multi-professional team co-ordinating the discharge of babies with on-going special needs both in the hospital and as part of the outreach service; will work in partnership with the parents, empowering them to meet their baby's on-going special needs following discharge and the continuity of care in the community and home setting
- The post holder is required to teach and supervise families and other nursing and non-nursing staff and be a resource person within the unit for discharge planning. The post holder is required to have flexible working hours and to deputies for the Lead Neonatal Critical Care Community Outreach Nurse where required.
