You will be part of a team of 7 Navigators, supported by the Macmillan Patient Support Supervisor.You will have an opportunity to make a real difference to the experience of cancer patients and their families while enhancing your support and communication skills. This is a unique role that provides direct telephone contact with our patients as well as the chance to liaise with both clinical and administrative teams internally and externally.The ideal candidate will have experience of working with or supporting patients or clients with complex holistic needs, a good understanding of cancer and advanced communication skills. Previous NHS experience would be an advantage but not essential.We are advertising 1x substantive role.

Main duties of the job

  • This role will support the coordination of care for post-treatment cancer patients through the National Cancer Survivorship agenda. This role will be focussed around the cohort of patients who are self-managing, and will support them in navigating the complex health and social care system following treatmen
  • The post holder will work closely with the Multi-Disciplinary Team (MDT), led by the MDT Lead Manager. The post will work closely with the Oncology, Surgical and Palliative Care teams to coordinate care across the Trust for patients with cancer. The post will closely link with the Clinical Nurse Specialists (CNSs) for each tumour group. They will work with patients and their carers on a daily basis
  • In addition, the post holder will be expected to develop and maintain effective working relationships with the voluntary and community sectors to support patient care across the health and social care continuum
