Senior Lead Anticoagulation Pharmacist

Job summary

We are looking for a dynamic, organised and highly motivated individual to be part of the haemostasis and thrombosis team with the clinical haematology department.

This new role will be focus primarily on thrombosis prescribing and safety as well as taking part in anticoagulation clinics and specialised care of thromboembolic disease in the context of malignancy. A key aspect of the role will be the implementation of a streamlined anticoagulation discharge pathway and follow up clinic across all three of the hospitals within our trust, Hammersmith, Charing Cross and St Mary's Hospitals. We expect the post-holder to take a key role in liaising with ward pharmacists and teaching and education in this context.

The post holder must be comfortable working in a multidisciplinary team, have excellent communication skills, show high attention to detail and be able to work under pressure to meet tight deadlines. They must have previous experience of working in the NHS and experience in specialist haemostasis and thrombosis care. Most importantly, they must be passionate about delivery high quality patient care. Main duties of the job

  • To lead, deliver, develop and evaluate clinical pharmacy services relating to anticoagulatio
  • To provide expert advice on pharmaceutical matters, undertaking risk management and ensuring compliance with medicines legislation
  • To facilitate and support the safe, efficient and cost effective use of medicines used within the anticoagulation service
  • To support the anticoagulation and related red cell haematology services through direct clinical care commitments for prescribing and counselling at the designated anticoagulation clinics and rounds, and provision of expertise at the relevant specialist MDTs
  • To manage, develop, train, motivate and encourage staff working within the team and support education and training of the MDT across the Trust.
