Job purpose

To deliver high quality, effective and individualised nursing care that is compassionate, caring, and in line with evidence based professional practice.

Communication and relationships

Possess excellent communication skills and be able to form a positive, therapeutic relationship with people who are experiencing acute crisis or distress in the context of partnership and trust

Clinical and patient care

Support the enhanced care team to deliver personalised therapeutic direct patient care in collaboration with the multi-disciplinary team in that area.

Demonstrate an enhanced understanding of signs and symptoms associated with mental ill health, how this may present in patient behaviour, such as through risk, and how to respond appropriately.

Ensure safe and effective enhanced care of complex patients through assessment, planning, implementation and evaluation of care.

Identify and intervene where circumstances contribute to an unsafe environment for patients and staff and bring these instances to the attention of the nurse in charge of that area and enhanced care team members.

Work autonomously within the parameters of the role to deliver quality nursing care aligned to the principles of recovery.

Demonstrate an in-depth knowledge of and understanding of the fundamentals of the Mental Health Act, Mental Capacity Act, DOLS/LPS and other legislation applicable to the practice area, the application to practice and how to ensure adherence and how they are implemented in practice ensuring adherence to legislation at all times.

Act as an advocate for patients within the department, to ensure a patient orientated, recovery approach to the delivery of care is upheld and standards within statutory, regulatory and clinical guidance is met.

Respect, and within the resources available, accommodate the cultural needs of patients to ensure high quality care and a good patient experience, with respect and regard for their customs, religious beliefs and personal doctrines.

Monitor patients progress, work towards safe and timely discharge plans and ensure barriers to discharge are identified and acted on appropriately

To be an effective communicator with patients/carers/families regarding treatment and management.

Ensure the timely and accurate monitoring and recording of enhanced care in accordance with Trust policy and approved documentation templates on the electronic notes system.

Leadership and Managerial

Organise ones own work, ensuring the most effective and therapeutic use of time.

Provide leadership, support and guidance to staff in the planning, delivery and evaluation of individualised care, demonstrating specialist knowledge, high standards of clinical practice, and giving advice when necessary.

Contribute to the development of this role and wider enhanced care team through monitoring of clinical standards, participation in audit, and highlighting ways to improve standards according to changes in evidence, patient need, and feedback from patients, family/carers and staff.

Contribute to the provision of a seamless, high quality service whilst the patient is under enhanced care, by enhancing the interface between ICHT staff, patients, liaison psychiatry, community mental health teams staff, and others involved in the provision of care.

Education, Training and Health Promotion

Assess the needs of patients, relatives and carers and provide them with information as requested

Act as a role model and mentor to nursing assistants, pre-registration students and nurses undertaking supervised practice and return to nursing programmes

To contribute to the development and maintenance of a positive learning environment for colleagues, patients and visitors.

Be wholly accountable for practice in line with the NMC code of professional conduct and taking every reasonable opportunity to sustain and improve knowledge and professional competence.

Remain up to date with current developments in nursing and ensure evidence based practice

Develop new skills and knowledge pertaining to the speciality as advised by the Mental Health Matron and/or Lead Nurse for Mental Health.

Conduct oneself in accordance with the NMC Code of Conduct and Trust policies

General Responsibilities

Policy and service development

Under the supervision of the team leader work on delegated projects related to local policy and service development.

Promote high quality clinical care, and assist in the development of the enhanced care team, through the use of evidence based practice and monitoring of clinical standards, including data collection.

To work together with the ward team to ensure effective use of resources and adequate stock levels.

Have and demonstrate standard basic keyboard skills

Implement safe moving and handling techniques

Meet the physical requirements of the post subject to any requirements for the Trust to make reasonable adjustments for the post holder.

Actively manage own annual leave in line with Trust and local Policy and Procedure.

Participate in the programme for clinical supervision as required.

Information resources

To be involved actively in maintaining data quality on PAS or similar systems in the future.

Undertake relevant training for electronic information systems in place & under development.

Research and development

Demonstrate and understanding of the importance of evidence based practice through holistic approach to nursing care and day to day clinical endeavours.

Participate in the collection of audit and research data as required.

At first under supervision, participate in conducting nursing audit to achieve the agreed standards.

Participate in Trust wide research awareness sessions for nurses and midwives and recommend innovations for clinical practice.

Freedom to act

At preceptorship and foundation gateways: required to use a degree of initiative and deal with matters independently where they can, for example a postholder may be required to deal with patient enquiries which are generally routine, but would refer non-routine enquiries to others (level 2).

At second gateway: required to act independently within appropriate clinical/professional guidelines and refer to their manager when necessary (level 3).

Person Specification

Qualifications

Essential

• NMC Registered Mental Health Nurse

Desirable

• Educated to degree level
• Evidence of continued professional development
• Nationally recognised preceptorship/ mentorship qualification

Experience

Essential

• Experience of working as an RMN at a Band 6 level
• Have an excellent understanding and experience of working with patients who have experienced psychological trauma demonstrating trauma informed care.
• Willingness to work across departments, including A&E.
• Understand the importance of maintaining patient safety and minimising risk in accordance with Trust policy and procedure.
• Knowledge and experience of working with patients in an inpatient mental health setting.
• Knowledge and experience of commencing and downgrading enhanced observations to high risk patients.
• Experience of working with patients who may be confused or present with challenging behaviour.
• Maintaining patient safety and minimising risk in accordance with Trust policy and procedure.
