Job summary

You will work with a team of physiotherapists including clinical lead physiotherapist, Rotational Band 6 physiotherapists, Rotational Band 5 physiotherapists, Band 4 Rehab assistant and Band 3 physiotherapy assistants.You will manage a clinical caseload using an evidence based client centred approach to assess, plan, implement and evaluate interventions. You will be responsible for developing and training junior staff and students.You will work closely with the specialist nurses, and other Allied Health Professionals to achieve service and Trust objectives. We work on a integrated therapy model and promoting skill sharing among the professions.

The post will attract an effective communicator with specialist acute and rehabilitation skills to work with older adults with complex needs, expertise and experience working with multi-disciplinary teams and a flexible problem solving approach at a senior level. The role will combine clinical, educational, research, service development and line management skills to support the Multi-disciplinary services.You will have opportunities to be involved in management roles, such as recruitment and retention, sickness monitoring and complaints management. We strongly support Continuing Professional Development and you will be supported in the role by the Clinical Lead Physiotherapist

Main duties of the job

  • To be professionally and legally accountable for all aspects of own work, including management of patients in your care.
  • To provide comprehensive and specialist physiotherapy assessment and diagnosis for patients with a range of highly complex presentations, by utilising advanced clinical reasoning, evidence-based knowledge, investigative and analytical skills and specialist manual assessment techniques.
  • To work both independently and within a team structure, managing the admission, discharge and onward referral to own and team caseload.
  • To formulate and deliver an individualised Physiotherapy management programme based on advanced knowledge of evidence-based practice and a wide range of specialist treatment options.
  • To work collaboratively within the MDT to deliver therapy programmes. To undertake a key worker role within the MDT to ensure co-ordination of patient episode of care and safe and effective discharge into the community.
  • To take a lead at clinical management meetings to ensure the delivery of a well co-ordinated multidisciplinary service and integration of physiotherapy into the treatment programme. This will include discussion of patient care, patient progress and involvement in discharge planning.
  • To co-ordinate the provision of physiotherapy reports including discharge summaries, for the multidisciplinary teams, GP's and community services

Desirable

  • Membership of CSP special interest groups relevant to speciality
  • Relevant Postgraduate clinical qualification

Experience

  • Significant relevant, recent clinical experience in the management of patients with age related conditions, patients with long term conditions requiring hospitalisation.
  • Experience of multidisciplinary goal planning, development of care pathways and using outcome measures

  • Experience of service development/change management
