Chelsea & Westminster NHS Foundation Trust invites applications for an exciting role in our CNS team for a Colorectal Clinical Nurse Specialist.

We are looking for an experienced Nurse who is committed to delivering excellent patient care and who takes pride in what they do and will work within the colorectal nursing team to improve and enhance the patients' journey. As an experienced nurse you will work alongside nursing and medical colleagues in surgery and oncology to facilitate the best experience and care for individuals with colorectal disease and suspect or confirmed diagnosis' of colorectal cancer.

The post holder will contribute to the on-going role in the development of the service for patients with Colorectal disease and Colorectal cancer. This will include developing protocols; setting up; and running a nurse led clinics (face to face/Virtual/telephone) for triage and follow up care which will include advanced assessment skills.

A background in colorectal nursing, or general surgery would be advantageous to the applicant. Excellent communication skills are essential, an ability to prioritise workload, whilst working as part of a team is also essential.

Main duties of the job

The Clinical Nurse Specialist (CNS) functions within a field of practice that focuses on the needs of the patient and encompasses interaction with others in delivering nursing, health and social care.

The CNS promotes excellence in nursing practice through the integration of expert clinical practice, education, consultancy, acting as a resource and commitment to nursing research. The post holder will work closely with the Urology CNS team across the patient pathway.

The post holder will contribute to the on-going role in the development of the service for patients with suspected or confirmed colorectal cancers. This will include developing protocols; setting up; and running a nurse led clinics (face to face/Virtual/telephone) for assessment and follow up care.

The post holder will also participate in the education of all disciplines of staff working with this patient group and act as an expert resource to other professionals involved in the care of these patients. They will work to ensure that cancer specific guidelines are adhered to and Department of Health standards are maintained.

The post holder is expected to develop this role in accordance with the framework for Nurses in Specialist Practice as identified by Hamric, Spross & Hanson and the Trust's Competency Framework for Specialist and Advanced Practice Roles.

  2. To demonstrate specialist skills and knowledge through expert nursing practice.
  3. To liaise with all relevant members of the multidisciplinary team in the care and support of individual patients including GPs.
  4. To maintain a high profile in the clinical area, acting as a role model for other health care professionals.
  5. To facilitate individualised patient care based upon the principles of evidence based practice.
  6. To act as a liaison nurse between investigations until patient is diagnosed with cancer or referred back to GP.
  7. To hold a nurse led clinic, including the development of written protocols based on national standards, triaging or following up suspected colorectal patients (benign and cancer), arranging investigations, reviewing results, diagnosing and referring back to a medical team as appropriate.
  8. To refer patients diagnosed with colorectal cancer for on-going support.
  9. To work in close collaboration with all members of the MDT.
  10. To provide professional clinical leadership and expertise to other nursing staff.
  11. To work as part of the Colorectal Clinical Nurse specialist wider team, rotating responsibilities to ensure cover of the entire patient pathway including post diagnostic care and support.

  1. To promote patient/carer independence through the provision of relevant teaching and support.
  2. To participate in and organise study days/education programmes.
  3. To develop resource and teaching packs for use by patients and/or staff in the clinical and educational setting.
  4. To embed patient and public involvement within the sphere of practice, seeking and acting on patient feedback of the triage assessment service.
  5. To ensure the provision of appropriate written information is available to patients and staff.
  6. To participate in the Trusts on-going educational programmes.
  7. To liaise with other organisations, charities and agencies dedicated to colorectal cancer.

Research/Audit/Quality

  1. To utilise up-to-date research findings in the delivery of patient care and to disseminate this knowledge to all members of the multidisciplinary team.
  2. To participate as appropriate in relevant research studies and audit findings and disseminate these findings to clinical practice as appropriate in order to develop best practice.
  3. To identify opportunities for nursing research within own specialist area and carry out nursing research appropriate to the speciality.
  4. To regularly audit service including eliciting the views of patients, carers and professionals ensuring patient feedback is incorporated into developments in the service.
  5. To attend relevant departmental and Directorate and network meetings relating to clinical governance and to contribute to the clinical governance agenda.
