Job summary
The post holder will work as part of the wider palliative care nurse specialist team and be responsible for providing specialist/complex advice and support to all members of the multi-professional team in the care of patients with a life limiting disease who are experiencing symptoms that require palliative care. Provide an effective and supportive service by giving specialist advice to our patients and those important to them with regards to the disease process, treatment interventions and symptom management. Manage and prioritise a caseload independently.
Main duties of the job
Work collaboratively with our Primary Care colleagues, hospices, community palliative care teams, social services, care homes, district nursing teams and other agencies for example the voluntary organisations. Support Northwest London, Pan London and National palliative care strategic planning to provide a seamless integrated pathway for our patient group and those important to them.
Participate in the on-going seven day face to face palliative care service.
Participate in on-going service development through audit, research and education.
Participate in the wider End of life care strategic education programme within the Trust.
Provide strong leadership, role modelling and guidance to junior ward staff and colleagues.
