Job summary
We are looking for an enthusiastic, committed and motivated Band 5 nurse (part time 0.6 WTE) to join our friendly team in the discharge lounge at West Middlesex hospital. This role involves Staff actively pulling patients for discharge from wards at the earliest opportunity to improve patient flow throughout the hospital. Staff must be able to carry out full manual handing duties. Good communication and effective interpersonal skills are essential along with recent experience working in an acute hospital setting
Main duties of the job
Main duties
To liaise with the wards and clinical site manager and actively pull patients identified as confirmed discharges that day to the lounge throughout the day.
To ensure patients receive ongoing care and support during their stay in the lounge.
To liaise with pharmacy and transport services to ensure early and safe discharged home or to another care setting as per their plan.
Ensure patients/carer understand all discharge medications.
Initiate appropriate care in an emergency situation.
To maintain safety of all patients in the lounge.
To ensure all patients are discharged from Cerner in a timely manner.

- To co-ordinate and facilitate patient transfers from all adult wards, clinics and ED to the discharge lounge and educate staff of appropriate patients
- Provide and maintain all nursing care as needed for patients within the discharge lounge.
- Develop and maintain a positive customer service focused environment to ensure excellent patient experience during the discharge process
- Support the Trusts goal to reduce length of stay and discharge initiatives such as 2 before 12 and our 4 hour ED standard.
- Develop and maintain effective working relationships and communication with the pharmacy department to ensure that patients waiting for their medications are not delayed unnecessarily.
- Develop and maintain strong working relationships and communication with the discharge team, clinical site managers and transport team.
- To ensure smooth, safe and effective patient transition from hospital to patients discharge arrangements, appropriate transport is booked and follow up arrangements are made.
- Share discharge performance with deputy head of site and discharge manager.
