Resident Pharmacist - Band 6

We currently have an exciting opportunity for a pharmacist to join our Residency Team to provide a 24-hour operational and clinical service to meet the demands in a busy hospital environment at Chelsea and Westminster Hospital NHS Foundation Trust (Chelsea & Westminster Hospital Site only).
As a Resident Pharmacist you will work as part of a shift system delivering a pharmacy service during nights, evenings and weekends. There is also a training and development opportunity to undertake the Joint Programs Board / ULC School of Pharmacy - Post Graduate Diploma in Pharmacy Practice for which Chelsea & Westminster Hospital NHS Foundation Trust is an accredited centre.

Main duties of the job
The post holder effectively participates in the provision of Clinical and operational pharmaceutical services to Chelsea and Westminster NHS Foundation Trust (Chelsea & Westminster Site) under appropriate supervision for grade and experience rotating through various specialist areas on a 4 month basis.
They also provide an out-of-hours resident pharmacy service on a rostered basis.

- Participate in each 4-month rotation as per the rotas and complete competencies for each section as defined. For each rotation there will be a designated supervisor and structured package of training. Rotations include:
- Dispensary
- HIV & GUM
- Technical Services
- Medicines Information
- Discharge Support
- Cover
- Medicine
- Surgery
- Paediatrics
This will involve the following:
- Effective participation in the provision of clinical trial medication, including preparation, dispensing and accurate documentation.
- Participate in staff induction and training for junior pharmacy staff.
- Participate in service audits as required.
- Effectively contribute to the production of Standard Operating Procedures (SOPs) for the appropriate section.
- Maintain high quality of data input of patient information into the pharmacy computer system in order to ensure excellent labelling standards for dispensed medicines
- Accurate dispensing and checking of all prescriptions, including controlled drugs, in a timely fashion for inpatients, outpatients (including A&E), day case and discharge medication.
- Provide effective and appropriate advice to patients on medication use in order to ensure their understanding of their medication regime
- Provide oral responses to medication queries generated by health care professionals.
- Team lead and supervise, on a day to day basis, as requested by the appropriate section head
- Undertake specific projects in accordance with objectives set.
- Deal with the procurement of medicines when required especially in emergency or out-of-hours situations.
- Be responsible for providing a clinical pharmacy service on a rotational basis to agreed groups of patients:
- Review prescriptions for accuracy and legality and identify actual or potential problems.
- Ensure the effective supply of medicines suitable for use as an inpatient and appropriately labelled for discharge if applicable.
- Ensure the effective outcomes of treatment with medicines
- Monitor patients for potential and actual adverse effects of their medicines
- Take steps to ensure patients understand the purpose of their medicines and deal with any practical issues that may prevent the optimal use of their medicines
- Support the planning of patient discharge with respect to medication, including the transcription of requests for dispensing.
- Demonstrate professional accountability to patients.
- Participate in the provision of pharmacy services outside normal working hours, single-handed, on rotational basis following the current shift pattern. The resident pharmacy service is provided for Chelsea & Westminster Hospital.
- Support to medical staff, patients and other healthcare professionals.
- Prescribing issues - corrections of omissions, advice on dosing and product selection, assessment of medication as possible cause of admission due to adverse effect or sub-optimal dosing.
- Medication histories - accurate assessment of patients current medication use in order to support decisions about continued care
- Administration - information regarding the administration of complex medications e.g. intravenous drugs requiring dilution, special handling requirements etc.
- The dispensing of prescriptions for newly admitted patients if appropriate in order to avoid doses being missed.
- The dispensing of take home medication in order to support appropriate management of beds.
- The dispensing of sterile products if appropriate.
- Initiation of drug recalls of defective or clinically inappropriate medicines, according to the local procedure in response to the MHRA, in order to safeguard patient care
- Accurately record all calls using the standard on call form.
- Attend Resident Pharmacist Team meetings on a weekly basis unless absence is agreed with the Resident Pharmacist Team co-ordinator.
- Make appropriate use of the back up system, which provides the resident pharmacist with a senior member of staff on call at all times for advice
- Prioritise all the above complex demanding workload in order of urgency (as will be working alone).
- Ensure that a written record of Continuing Personal Development (CPD) is maintained for review at appraisal.
- Attend and participate in departmental meetings and participate in departmental quality initiatives
- Follow legal, ethical, professional and employers codes of conduct
- Demonstrate awareness and commitment to the Trusts Clinical Governance Agenda

- Registered as a practising pharmacist with the GPhC
