Job summary
Due to planned expansion of our ICUs on both sites, an exciting opportunity has arisen at the Chelsea & Westminster and West Middlesex NHS Foundation Trusts.
We require a capable, innovative and enthusiastic pharmacist to join our clinical team and work cross site supporting the pharmacists for ICU.
You are required to have recent hospital experience as a clinical pharmacist practitioner, demonstrating appropriate levels of competency e.g. able to show consistent achievement in all areas outlined in the Advanced Level Competency Framework (Delivery of Patient Care, Problem Solving and Personal). You are expected to be familiar in the clinical screening of ICU drug charts and will have covered ICU as a band 7 pharmacist or above.
All posts will undertake clinical audit, financial management, deputise or provide cover for the relevant Lead Directorate pharmacist(s) and provide pharmaceutical care of patients within the Planned Care division.
You will be a good team player with a keen interest in patient focused care, have the ability to prioritise and meet deadlines.
You will participate in multidisciplinary ward rounds on the Intensive Care and be an integral member of the team contributing to service development and clinical governance.
Main duties of the job
- Be the pharmacy representative on ICU ward rounds as delegated by the Lead Directorate Pharmacist for Imaging & Anaesthetics.
- Participate in the day to day directorate service and deputise for the Lead Pharmacists in I&A cross site as required.
- Support and supervise less experienced pharmacists in ICU.
- Actively contribute to improvement initiatives aimed at optimising medicines use in line with the Trust's key priorities.
- Actively be the pharmacy representative on the TPN ward rounds
- Provide clinical advice to the acute and chronic pain teams

- Clinical Practice
- Provide clinical pharmacy services to patients admitted to the Adult Intensive Care Unit (ICU) and other clinical areas of the Planned Care Division in accordance with the departments procedures and priorities as determined by line manager
- Review prescriptions for accuracy and legality and identify actual or potential problems.
- Ensure the effective and uninterrupted supply of medicines for use in ICU
- Take steps to ensure the effective outcomes for patients through treatment with medicines.
- Monitor patients for potential and actual adverse effects of their medicines.
