Senior Clinical Fellow in Regional Anaesthesia

Job summary

2 x Senior Clinical Fellow in Regional Anaesthesia required.

This role commences in February 2024 and is a 6 month post, with a possibility to extend to 12 months. We also accept ASAP start date.

The job is aimed at post-fellowship registrars who are nearing or have completed their Certificate of Completion of Training in Anaesthesia. The post aims to prepare fellows for a consultant anaesthetist post with a specialist interest in regional anaesthesia.

Main duties of the job

The fellow will be allocated an educational supervisor on induction. Learning outcomes will be defined at the beginning of the placement. Fellows will meet with their supervisor at 3 months to ensure that their educational needs are being met. Final sign-off will be at the end of six months subject to evidence such as a satisfactory log-book, appropriate workplace-based assessments and engagement with agreed clinical projects.

Job description

Job responsibilities

It is expected that fellows will be allocated a minimum of 50% of surgical lists with a regional anaesthesia component. These may be a mixture of supervised and solo lists.

Once sufficient training has been received Fellows may be required to provide daytime roaming regional anaesthesia input for situations of acute pain for example patients with fractured ribs or fractured neck of femur.

On-call commitments will be on a 1:8 basis. This may be either covering the senior registrar bleep or the labour ward bleep depending on departmental staffing mix at the time of placement. The senior registrar oncall covers the general emergency theatre (general surgery, plastics, paediatrics, neonatal surgery and burns). Responsibilities include directly supervising a CT1-2 anaesthetist and providing additional support to the labour ward anaesthetic registrar as required.

REGIONAL ANAESTHESIA OPPORTUNITIES

Chelsea & Westminster Hospital performs a wide variety of surgery which is amenable to the benefits of regional anaesthesia. These include:

  • Hand Management Unit

We have 8 hand surgeons and therefore multiple operating lists each week. We perform a variety of blocks including axillary, supraclavicular, infraclavicular brachial plexus blocks and peripheral upper limb blocks. You will have the opportunity to gain experience in managing awake patients for hand surgery lists.

  • Shoulder/ elbow surgery

We have 2 operating lists each week where you may have the opportunity to perform interscalene and infraclavicular blocks.

  • Lower limb surgery

We have 7 lower limb surgeons and this will provide experience of femoral, fascia iliaca, adductor canal, popliteal and ankle blocks.

  • Catheter based techniques

Multiple opportunities exist to learn catheter techniques, including rectus sheath blocks for major abdominal surgery & infraclavicular catheters for elbow surgery. We have also recently developed a new pathway for the management of rib fractures. As the Fellow you will heavily involve in the anaesthetic management of these patients, including the insertion of serratus plane catheters.

  • Central neuraxial blocks

You will have the opportunity to learn ultrasonography for central neuraxial blockade.

  • Paediatric regional anaesthetic blocks
  • Paravertebral blocks

Through our links with the Royal Marsden Hospital you may be able to attend some theatre sessions to gain experience in placing these blocks.
