Job summary

The Liverpool University Hospital NHS Foundation Trust has an exciting opportunity for an enthusiastic nurse to join our dynamic Complex Discharge Team. This position is well suited to an individual that is looking to advance and change their career path while gaining hands on experience in discharge planning and clinical utilization in a supportive workplace.

You will be working closely with the multi-disciplinary team and community partners to proactively plan and arrange the discharge of a diverse group of patients. You will be able to enhance your own clinical knowledge as well as provide expert knowledge to your peers.

The Band 6 Complex Discharge Nurse is an essential post within the Integrated Discharge nursing workforce. It performs critical patient-facing, support, and coordination functions that directly impact the safe delivery of discharge pathways, access to services, and service continuity.

Main duties of the job

Responsible for ensuring patients have a timely and appropriate dischargePlan, with support will act upon any delays quickly to promote a safeand effective discharge.

To ensure that patients and relatives have a named individual to assistthem to be involved in decisions about their care and how they willbe affected.

To Act as a support for ward staff, facilitating the patients dischargefrom hospital, working alongside hospital

The key tasks undertaken by the post include:

Completion of descriptive Transfer of Care document in a timely manner of NCTR status being declared

Effective Parallel planning and gathering of social histories following a strengths-based approach to identify early barriers to discharge and address

Accurate and contemporaneous updating of OPTICA patient tracking system to drive discharge and report an accurate sit rep locally and nationally

  • You must have at least 2+ years post registration experience within an acute setting working as a Registered Nurse.
  • Knowledge of Discharge Planning and Pathways is essential.
  • To review all allocated patients regarding the necessity to stay in hospital against criteria to reside on a daily basis.
  • Identify barriers or gaps for delays to treatment or discharge.
  • Demonstrate a knowledge of NICE, Map of medicine guidelines specific to the speciality the patient is assigned to.
  • Identify delays in diagnostics or treatment.
  • Use utilisation management system software package to record patient information, compliance with treatment and any variances to that plan.
  • To analyse data and information to produce reports of patient flow
  • Make presentations as necessary on relevant topic areas.
  • Work closely with Consultants, nurses, AHPs around management plans to consider alternative methods and venues for completing interventions out of hospital where this would be clinically appropriate whilst also minimising length of stay.
  • Resolve issues that are preventing patients who do not clinically require to be in an acute hospital bed.
  • To work with the ward staff, ward managers and Matrons to support them in their role of discharge planning and reducing length of stay.
