We are looking for a motivated, proactive individual with a strong attention to detail and good time management skills, to join our ward Department.
The candidate should have experience of working in a fast paced Department in a clinical role.
The role of the flow coordinator plays a key role in ensuring efficient patient flow by monitoring the patient's journey through without unnecessary delays.
This post is required to support patient flow and discharge management within the service, ensuring timely and coordinated discharges while facilitating effective communication between multidisciplinary teams (MDTs) and external stakeholders.
Optimising patient flow to improve bed availability and reduce delays.
Managing safe and timely discharges across Pathways 1, 2, and 3.
Providing essential MDT support by coordinating discussions and decision-making.
Facilitating Best Interest Meetings (BIM) to ensure patient-centred discharge planning.
This service will cover 08.00hrs-16:00hrs Mon- fri
Main duties of the job
