Job summary
We are seeking a highly skilled and motivated Advanced Clinical Practitioner to join our Clinical Haematology team. This role offers the opportunity to work autonomously and collaboratively within a multidisciplinary team, providing expert care to patients with a range of haematological conditions.
Working in the inpatient, outpatient and day case setting the successful candidate will assess patients, plan, deliver and monitor the care and management of clinical haematology patients.,
The successful applicant will be expected to demonstrate advanced clinical skills, participate in service evaluation and improvement, audit and research.
Main duties of the job
- Assess patients and plan, implement and evaluate care using specialist clinical knowledge and skills
- Review patients and provide care in inpatient, outpatient and day ward settings
- Contribute to multidisciplinary approach to patient management
- Provide education and support to patients, carers and colleagues
- Participate in audit, research and service development within the Haematology service.

- Perform independent specialist assessment of patients to formulate a diagnosis, plan and implement care and arrange ongoing follow up as an advanced practitioner using highly developed specialist clinical knowledge advanced clinical skills.
- Independently manage a patient caseload providing expert specialist assessment, planning and evaluation.
- To lead the multi-disciplinary team in resolving patient problems, by the provision of new and innovative models of care
- Act as a role model and expert clinician providing in depth specialist clinical knowledge to colleagues through MDT meetings, for patients and carers / relatives.
- Act as an innovative, enthusiastic role model providing leadership, guidance and advice to staff on operational and professional issues promoting an open and honest and transparent culture.
- To improve the experience and outcomes of care for patients by taking the lead in investigations and treatments prescribed.
- To provide a consistent and personalised service by ensuring that the patients receive the best care, in the best place from the best person or team.
- To provide social, emotional and psychological support for patients from referral and throughout their treatment.
- To deliver expert specialist evidence based care including: development, assessment, planning, implementing, evaluating programmes of evidence based nursing care for a defined caseload.
- To make appropriate referral to consultants/other specialist/members of the multidisciplinary team in primary, secondary or tertiary care settings.
- To be aware of risk management issues both locally and nationally and develop strategies to address identified issues.
