General Practitioner with an Extended Role in Frailty (GPwER)

Job summary

Liverpool University Hospital NHS Foundation Trust wishes to appoint a General Practitioner with an Extended Role in Frailty based at its Aintree Site (Less Than Full Time 6 PAs). We are looking for dynamic and enthusiastic individuals to expand our geriatrics team and improve integration with community services. This is a vacant position in the Trust.

As the population ages there are increasing numbers of older people living with frailty who are presenting to ED departments. It is recognised that targeting this high risk group as early as possible during admission provides an opportunity to improve quality of care and optimise patient and service outcomes for a vulnerable population.

Main duties of the job

The post holder will work alongside other Geriatricians to develop innovative 7-day services for older people, both within the hospitals and the community. The post will include:- Sessional work in the community, working alongside the Community Geriatrician in post and supporting patients in their home environment, including those with barriers to attendance at hospital appointments.- The role will be variable and home visits will be co-ordinated to encompass new and follow up reviews utilising the Comprehensive Geriatric Assessment as the cornerstone of practice.- Undertake sessional work on the Frailty Assessment Unity (FAU), working alongside Frailty Consultants to deliver Comprehensive Geriatric Assessments to patients presenting at the front door.- Provide in-reach services to the Emergency Department. Although the number of patients will vary each session, the number would not exceed 8 patients.

  • Certificate of Completion of Training (CCT) or equivalent in general practice

Clinical Experience
