Job summary

We are delighted to offer an opportunity for an exciting Pancreatobiliary (PB) Advanced Endoscopy Fellow post for 12 months in a tertiary unit in our new, busy state-of-the-art endoscopy unit located in the new Royal Liverpool Hospital. The endoscopy unit is co-located with the Mersey School of Endoscopy, a National Training Centre and the Directorate of Gastroenterology and Hepatology.

Main duties of the job

The job plan is structured to provide dedicated hands-on tertiary level training in a wide range of PB endoscopy (ERCP, EUS and endoscopic necrosectomy). The successful candidate would be based at the new Royal Liverpool Hospital, part of Liverpool University Hospitals NHS Foundation Trust (LUHFT).

The fellow would be working under the supervision of the endoscopic pancreatobiliary team, comprising of 3 Consultant Endoscopists (Dr Jayapal Ramesh, Dr Howard Smart, Dr Khoon Sheng Kok) and 1 Consultant Interventional Radiologist (Dr Jonathan Evans). There are daily ERCP lists and 5 EUS lists/week, with 2 lists with cytopathologists present for ROSE. There would be additional training and exposure to other complex endoscopic procedures such as luminal stenting, duodenal EMR and ampullectomy. The post also requires participation in the medical pancreas clinic and service endoscopy lists. The post would be ideally suited to a post CCT or senior registrar looking to gain experience in PB endoscopy and medicine prior to a consultant post.

THE POST

The job plan is structured to provide dedicated hands-on tertiary level training in a wide range of PB endoscopy (ERCP, EUS and endoscopic necrosectomy). The Pancreas Unit

The Merseyside and Cheshire Regional Pancreatic Cancer Unit is based at the Royal Liverpool Hospital. It is a tertiary referral centre for the region, nationally and internationally, and one of the busiest units in the country. There are around 150-200 major pancreatic resections performed per annum for both cancer and benign disease. The RLH is a specialist referral centre for acute pancreatitis and its complications and receives over 200 external referrals for such patients each year.

There is a very high level of co-operation between the interventional endoscopy team, surgeons, radiologists, pathologists, research and nursing staff that enable the unit to perform interventions in a timely fashion. There is an increasing use of endoscopic ultrasound for both diagnostic and therapeutic indications for e.g. peripancreatic fluid collection drainages, bile duct and pancreatic ductal drainages. The candidate will be able to learn from and interact with a very proactive, world renowned team of experts. There is also potential to develop ideas for treatment and research into pancreatic pathology.

Multidisciplinary Team Meetings

There are weekly Pancreas MDT meetings on Wednesdays (Cancer/cyst) and benign MDT meetings (Tuesdays). The fellow will be an active participant in these meetings.

Research and Teaching

The fellow will be actively involved in research within PB endoscopy and medicine, taking opportunities to strengthen their CV with presentations in national/international meetings and publications.

The fellow will also be involved in the national EUS FNA courses and Nurses pancreatobiliary course (ENRICH). There is also potential to set up hands-on training courses in the future,

HPB Nurses

The PB fellow would be working closely and supporting the HPB Specialist Nurse and HPB Nurse Coordinator, vetting inpatient and outpatient referrals and reviewing patients if necessary. The appointee will have opportunities to evaluate the service flow, streamline good clinical practice; furthermore, any new ideas for patient pathways will be encouraged.

Service delivery

The post will also be utilised with commitment to service which includes diagnostic & basic therapeutic endoscopy, colonoscopy lists and inpatient lists. It is therefore essential that the applicant meet the JAG criteria for independence in both Gastroscopy and Colonoscopy. There are ample opportunities on the service lists to undertake endoscopy for GI bleeders.

On-call commitments

On call commitment will be purely gastroenterology and GI bleeders supported by on call consultants. On the weekends, the fellow would have in patient responsibilities for Gastroenterology and Liver patients which would include ward rounds, taking calls from outside hospital referrals and emergency endoscopy. The weekend on call is supported by ward consultant and endoscopy consultant who would otherwise be performing regular endoscopy sessions during the day. Emergency out of hours endoscopy is undertaken in theatre. Discussions are ongoing for a city wide support for emergency ERCP.

Endoscopy Unit

The successful candidate will be joining are one of the largest endoscopy units in UK that performs over 20,000 procedures per annum. It is a Bowel Cancer Screening Centre and offers the full depth and breadth of diagnostic and therapeutic endoscopic procedures. This includes HALO, ERCP, EUS, Capsule Endoscopy. The Royal Liverpool Endoscopy unit became the first in the country to perform STRETTA and first in the North of England to perform POEM.

The endoscopy unit moved to the new Royal Liverpool hospital in September 2022. The new unit comprises of 7 endoscopy rooms; there is a dedicated ERCP suite with an in-built C arm. There is also a dedicated EUS suite. It also boasts a fully functional A-V system for clinical, teaching and training purposes. The Unit operates a 7 day a week service. The Mersey School of Endoscopy is located adjacent to the endoscopy unit.
