Job summary

An exciting opportunity has arisen for a Band 6, Senior post anaesthetic care unit practitioner. The Theatre Recovery unit's within Liverpool FT University hospitals Trust include Aintree University Hospital, Liverpool University Hospital Dental Hospital and Broadgreen Hospital including the PECU/OPERTA Unit PMU on BGH site.

Liverpool Ft University NHS Trust have specialties, including Trauma, Orthopaedics, General surgery, Minor Colorectal, Breast, and laparoscopic surgery. We also have a 4 bedded Post Elective care unit and a Pain Management Unit which is part of the recovery unit at BGH. As part of the recovery service we recovery patients in ECT in Broad Oak Hospital.

The Theatre Recovery Unit's are situated within the theatre unit's across all the sites. The current staff are all highly skilled recovery practitioners responsible for the safe admission care and discharge of all patients from the variety of disciplines mentioned above to destinations such as Day case ward , PECU/OPERA and Wards within the hospital.

Main duties of the job

The main duties are to plan deliver and evaluate all aspects of patient care for the immediate post operative patient who have undergone surgery or a procedure within the Trust and the external arrears mentioned in the job overview.

Duties include:

Airway management.

Vital signs monitoring using non invasive and invasive systems, and be able to react to changes using NEWS scoring system.

Pain and nausea management using pain assessment tools and PONVS

Fluid balance monitor input and output and react to changes

Wound and drain care including VAC drains.

Pressure area care.

All aspects of personal care.

In the absence of the unit co-coordinator/team leader the successful candidate would be expected to assist in the day to day running of the unit ensuring effective communication with theatres and the multidisciplinary team. The successful candidate should possess significant recovery or equivalent skills, such as HDU/critical care. And have experience within these arrears. They must be able to work in all areas of recovery and PACU It is also expected that candidates possess either a enhanced recovery, high dependency or critical care qualification. Theatre recovery works on a 5 day working week and a Saturday early shift basis on the BGH site, therefore applicants must be aware that there are a variety of shift patterns including nights and Saturdays.

CLINICAL MANAGEMENT:

Deliver skilled practical care to all patients and lead in the monitoring, planning and delivery of individualised patient care, providing advice to staff as appropriate.

Able to demonstrate the ability to assess situations that arise in the peri-operative environment.

To participate, as required, in out of hours cover and rotation onto night duty.

Liaise with Unit Co-ordinators/Team Leader to support appropriate stock management and promote efficient and effective use of resources by continually examining departmental procedures and products used.

Liaise with Unit Co-ordinator, Team Leader and equipment lead to ensure all equipment and instruments are maintained in accordance with manufacturers guidelines, document and report immediately any faults as they occur.

Participate in unit meetings/team feedback to staff as required.

Ensure effective communication with line manager in day to day running of the department acting on own initiative as appropriate.

Ensure rigorous adherence to infection control practices including hand hygiene and universal/standard precautions.

Post holder is required to prioritize and manage workload as an individual and part of the MDT and act up for team leader/Unit Co-ordinator in his/her absence.

Support/promote the implementation of Evidence Based Practice and participate in clinical audits as required.

Participate in setting and maintaining agreed standards of patient care ensuring that said practices are in accordance with departmental/hospital policy.

Input patient data onto the ORMIS system to ensure that accurate patient records/activity are recorded.

Personal/Professional Development:

Identify personal and professional needs for career development of self and junior staff via the Trust appraisal programme and the NHS knowledge and skills framework. Take a lead role in the Personal Development Review for junior staff.

Actively support students and professional staff with the mentor/preceptorship role assessing their practical skills and knowledge with courses and clinical competencies as required.

In consultation with the Training Co-ordinator take a lead role in the training and development of other team members, newly appointed staff and students.

Support staff undergoing orientation and induction specific to the clinical area.

RECOVERY DUTIES:

Receive and identify patients post-operatively along with all relevant documentation from anaesthetist and theatre practitioner, ensuring all information pertaining to the patients peri-operative episode is correct, as per departmental guidance.

The practitioner is competent to deliver expert, skilled care in a specialised area.

Ensure patients receive appropriate pain and nausea management whilst in recovery, including the initiation of PCA following Trust/departmental guidance.

The practitioner is competent in the management of I.V. lines/ PCAs, arterial lines, central lines, epidurals and medication administration as per Trust Guidance.

The practitioner is competent in airway management. This is inclusive of managing patients in the immediate post-operative phase who has e.g. a laryngereal mask or and Gudel airway in place. The practitioner possesses the knowledge to assess when a patient conscious level warrants removal of either of the artificial airways referred to.

The practitioner reports immediately, any adverse changes in the patients condition or any other aspect of concern as identified in the MEWS score as appropriate. The practitioner is competent in the recognition of primary post-operative complications and understands the need to act in a timely manner.

Practices nurse led discharge i.e. discharges patients from the Recovery Unit to the ward area. The practitioner will implement a care plan for the continuing care of patient on the ward and provide a safe and effective handover of the patients peri-operative journey to the ward nursing staff, as per departmental guidance.

The practitioner completes the recovery competency template through the stages novice, proficient and expert.

On occasion there may be a delay in a patients discharge from the Recovery Unit e.g. ward bed not available and/or staff on ward dealing with a crisis. In these circumstances the patient may have an extended stay in the recovery unit and in some instances an overnight stay. The senior practitioner will be required to manage the situation as appropriate communicating with the MDT patient and relatives as appropriate updating ORMIS and IPM.

If a patients post-operative status warrants transfer to a Critical Care bed the practitioner will work with/support the anaesthetist responsible for the patients care until the patient is discharged from the unit. In the absence of the anaesthetist the practitioner will use their expert knowledge, clinical and analytical skills at the bedside and alert the anaesthetist, should the patients condition deteriorate and warrant medical opinion/advice.

The post holder will, on occasion, be required to work in traumatic and emotive environments.
